  • 5:00 PM Position Summary MEDRITE is seeking a knowledgeable and mission-driven Social Care Network (SCN) Screener to join our growing Clinical Services team.
This role is central to MEDRITE's participation in New York State's Health-Related Social Needs (HRSN) initiative under the
NYHER 1115
Medicaid Waiver. The SCN Screener is responsible for conducting standardized HRSN screenings for Medicaid members, identifying unmet social needs—including food insecurity, housing instability, transportation barriers, and related factors—and initiating the workflow to connect patients with appropriate resources and services. The ideal candidate brings direct experience working within a Social Care Network, managed care, or Medicaid insurance environment and understands both the clinical and administrative dimensions of this work. Key Responsibilities HRSN Screening & Member Engagement Administer the AHC Health-Related Social Needs (HRSN) standardized screening tool to Medicaid members during in-person or telephonic encounters. Accurately document screening results and member data in the SCN IT platform and/or electronic health record (EHR) system in a timely manner. Conduct member eligibility reviews to determine qualification for Enhanced HRSN Services, including nutrition, housing, transportation, and care management. Engage members with empathy and cultural sensitivity, ensuring a person-centered experience throughout the screening process. Identify and flag members with urgent or complex social needs for escalation to the Social Care Navigator or care team. Documentation & Compliance Enter all screening activity into required platforms, ensuring data integrity and compliance with
NYS DOH SCN
data submission requirements. Track screening completion rates, eligibility outcomes, and referral statuses to support program reporting and reimbursement. Maintain working knowledge of SCN billing and reimbursement protocols, including documentation requirements tied to screening episodes. Adhere to HIPAA, Medicaid compliance standards, and organizational policies governing member data and confidentiality. Team Collaboration & Program Support Partner closely with Social Care Navigators, Care Managers, and clinical staff to facilitate warm handoffs and continuity of care. Participate in team huddles, training sessions, and quality improvement initiatives. Assist the SCN Lead with outreach to eligible Medicaid members for proactive screening and enrollment. Stay current on SCN program updates, screening protocols, and community resource availability. Qualifications Required Associate's degree or higher in Health Sciences, Human Services, Social Work, Public Health, or a related field. Bachelor's degree strongly preferred. Minimum 2 years of experience in a Social Care Network, Medicaid managed care, health insurance, or community health setting. Demonstrated familiarity with HRSN screening tools, SDOH frameworks, or Medicaid member engagement. Experience with electronic health records, care coordination platforms, or SCN IT systems (e.g., Unite Us, Healthix, or similar). Strong attention to detail and ability to manage high-volume documentation accurately. Culturally competent; ability to engage diverse, underserved patient populations with compassion and professionalism. Preferred Prior experience working directly within an SCN ecosystem, IPA, or health plan conducting HRSN screening or eligibility assessment. Knowledge of
NYS 1115
Medicaid Waiver and Enhanced HRSN service categories. Bilingual proficiency, particularly Spanish, strongly preferred. Familiarity with eMedNY or Medicaid billing workflows.
