Responsibilities/ Job Duties:
Inbound & Outbound Coordination:
Serve as the primary point of contact by promptly and professionally handling incoming calls, emails, and faxes regarding placement and treatment inquiries. Make necessary outbound calls to hospitals, discharge planners, and case managers to finalize placements.
Referral Pipeline Management:
Build and manage data records for new referrals. Routinely monitor dashboard updates and communicate status changes efficiently across internal teams.
Relationship & Liaison Management:
Cultivate exceptional, trust-based relationships with external medical partners (social workers, physicians' offices, hospitals) and internal teammates across different geographical locations.
High-Volume Data & Metric Tracking:
Actively manage a standard call volume of 40 to 50 calls per day once fully trained, maintaining high work efficiency and low post-call documentation times.
Database & Process Improvement:
Accurately input patient data forms from the field into internal databases and proactively contribute insights to improve standard operating procedures and CRM tracking.
Emergency & Feedback Support:
Provide community relief during emergency situations by facilitating urgent patient placements and conducting welfare checks. Administer telephone and email surveys to continuous improve service delivery.
Compliance & Policy Adherence:
Strictly maintain data confidentiality and safeguard Protected Health Information (PHI) in full compliance with HIPAA regulations .
Minimum Education and Experience Requirements:
High School Diploma or equivalent transcript verification required. Minimum of 6 months of related experience in a call center, customer service, or administrative environment.
Preferred Qualifications:
Healthcare Exposure:
Prior experience in medical admissions, healthcare scheduling, or medical office environments.
EMR/EHR Systems:
Familiarity with Electronic Medical Record software such as Epic, Allscripts , or similar data-housing platforms.
Telecom Experience:
Previous use of enterprise contact center platforms or telecom systems (e.g., CX1, Cisco Finesse ).
Knowledge of Specialty Care:
Basic knowledge of or a strong desire to learn about chronic diseases and related specialized medical care.

M-F 9:00am - 5:30pm
MST M-F 11
00am - 7:30pm
