The position plays an integral role in maintaining the integrity of the program and will require daily interaction with patients as well as external stakeholders and medical office personnel. The ability to protect confidential information is critical. Excellent communication skills, both written and verbal, are essential. Spanish or ASL proficiency is highly recommended. The project's primary mission is to increase to specialty care for West TN's uninsured population. Duties and responsibilities Process incoming and outgoing faxes and emails with patient information Discuss medical issues and potential eligibility with clinic provider Enter captured information into database and record patient encounters for reporting associated with Project Access West TN Track, manage, and process referrals from physicians, community-based organizations, and health care administrators. Work as a member of the team to coordinate patient care, communicating updates and follow- up instructions with patients along the way. Determine eligibility of patients, collect financial records from patients, and document any barriers to eligibility Communicate appointment information and other instructions to patients, via phone, email and paper mailings Follow up with and re-verify eligibility of patients as needed Qualifications Excellent written and verbal communication skills. Fluency, both written and verbal, in Spanish highly preferred. Requires a High School diploma. Four-year degree preferred. Current undergrad or grad students near graduation will be considered. Excellent customer service and organizational skills. SQL or other relevant database experience. Proficient in Microsoft Office suite. High attention to detail and deadlines is required as well as Exceptional relationship skills High adaptability, with ability to support multiple ongoing projects. Answering incoming calls and assisting other staff members as needed. Coordinate transportation and provide information regarding the insurance marketplace and other social services. Proven ability to communicate both in written and verbal forms with people from a variety of backgrounds.

Working conditions This job requires some night and weekend activities. Otherwise, work is typically conducted between 8:00 am - 5:00 pm Monday-Friday. Physical requirements Ability to lift up to 25 pounds.
