Job Description
Coordinator (Bilingual Preferred) Non-Exempt, Full-Time (30 hrs), Safety Sensitive, Minimum Starting Pay $13/hour (Negotiable DOE) Job Summary The coordinator will provide home visits and conduct intake processes with consumers, as well as perform clerical duties in the office. Coordinators will report directly to the office manager and will be responsible for monitoring consumers. Coordinators may be responsible for screening, training, and providing coverage for personal care attendants. Coordinators are generally scheduled to work main office hours (Monday-Thursday, 8am-4pm) and may answer after-hours calls and provide essential coverage to consumers when necessary. This position is Safety Sensitive. Coordinators are responsible for the direct care of consumers. Safety Sensitive positions are those in which performance by a person under the influence of drugs (whether prescribed or not) or alcohol would constitute an immediate or direct threat of injury or death to that person or another. Although medical cannabis may be legal in the state of Texas, safety sensitive positions may not be under the influence of this substance whether prescribed or not. Safety Sensitive positions are subject to reasonable suspicion alcohol/drug screenings for workers who appear to be impaired or may otherwise be in violation of the drug and alcohol policy. Essential Responsibilities 1. Coordinate and provide home visits to consumers on a monthly/quarterly basis or when requested 2. Conduct referral and intake processes with new consumers as required 3. Monitor and provide documentation on consumer safety and general physical health and wellbeing 4. Manage, schedule, and/or attend various meetings 5. Develop plans of care, agendas, action plans, and various documents 6. Update and submit records and gather signatures for documentation and billing purposes 7. Schedule staff; coordinate substitutes and/or provide caregiving duties when necessary 8. Screen, train, and provide feedback to caregivers 9. Quality Assurance; Audit consumer files as well as own work processes 10. Maintain systems (paper/electronic) for tracking consumer/employee information 11. Perform administrative functions such as data entry, file, draft correspondence, answer/return calls 12. Drive company or personally owned vehicle to consumers' residences to conduct visits, checks, or caregiving duties including medication/grocery pickup. (Mileage reimbursement for personal vehicle usage.) 13. Understand and comply with policies and procedures of funding agencies and Texas Health and Human Services 14. Understand and comply with Abarim Home Healthcare, Inc.'s policies and procedures 15. Perform other duties as necessary or as requested by the office manager or other persons of authority Physical Demands 1. Requires the ability to occasionally lift, carry, and pull up to 40 lbs. or more 2. Must have unlimited range of motion 3. Requires ability to sit for prolonged periods and work on a computer and/or phone 4. Must be able to drive a vehicle 5. Must be able to meet safety-sensitive requirements such as not using substances which may inhibit judgement, performance, or safety of self/others while on the job Core Competencies 1.
