  • Field Based) Join a Mission-Driven Team Supporting New Yorkers in the Community (• Health Home) We are seeking compassionate, organized, and highly professional Intake Specialists to conduct in-person member enrollment visits and assessments throughout assigned communities.
This role is ideal for individuals who enjoy meaningful face-to-face interactions, independent field work, and making a direct impact on vulnerable populations. This is a flexible per-diem position with strong team support, structured training, and competitive per-assignment compensation. Initial and on-going training & support are provided to all employees. Our team of experienced, knowledgeable but approachable colleagues and supervisors will provide support and guidance to assist you complete your daily tasks. Compensation Flexible per-diem pay (not hourly) Steady case availability — up to 50 visits/packages monthly Earn $250 per intake visit (approximately 3+ hours) Earn $100 per renewal/signature visit (approximately 3+ hours)
Bonuses:
$100 immediate onboarding bonus (complete onboarding within 2 weeks) $500 referral bonus (per successful hire) Key Responsibilities (including but limited to)
Member Intake & Enrollment:
Conduct face-to-face intake visits (remote intake is not permitted) Complete outreach and pre-visit documentation; Obtain required consent and care plan signatures Conduct comprehensive assessments, including behavioral health; psychosocial, social determinants of health Verify supporting documentation including: Identification, Insurance information Educate members on program services, rights, and responsibilities Identify urgent concerns and appropriately escalate when necessary Complete & submit timely and accurate post-visit documentation Ensure all enrollment packages are complete, accurate, and compliant Qualifications Able to speak and understand Chinese, Fuzhounese, or Cantonese New graduates are welcome — no prior experience required High school diploma minimum Health Home experience is a plus, not required Why Join Us Flexible schedule — no traditional 9-5 office hours Manage your own caseload Mileage and transportation reimbursement Structured training and ongoing support Strong supervisor and team support New graduates/no experience welcome!
