Job Description

Job Overview The Medical Records Assistant supports the accurate collection, organization, maintenance, and protection of resident health records within the long‑term care facility. This role ensures compliance with federal and state regulations, promotes efficient clinical documentation workflows, and assists clinical and administrative staff in accessing and managing health information. Duties Always demonstrate Luther Haven Values Communication with Residents, families, visitors, and coworkers using AIDET & Key Word principles. Listen attentively & observe both verbal & non-verbal communication. Assist MDS Coordinator and Care Coordinator as needed Utilizes exceptional communication skills with co-workers. Participates in committee meetings as assigned. Complies with the standards, policies, and procedures of Luther Haven. Maintains confidentiality of information regarding residents, families, health care personnel, and Luther Haven. Consistently coordinates daily work to achieve maximum productivity and efficiency. Answer phones and direct calls as necessary Handles telephone inquiries with courtesy and accuracy, receives information, and distributes messages as necessary. Monitor and maintain organization of resident medical records. Check and replenish forms weekly or as needed. Thin resident charts as needed. Ensure provider clipboards are ready for provider to sign off on orders. Put in physician orders into LTC EMAR, putting orders for labs and radiology into computer. Assist with making reservations for transportation for appointments out of the building. Assist with making resident appointments Coordinate discharge and admission records. Preparing necessary paper forms Computer entry of physician orders, medications, diagnosis, treatments, payment information, and all necessary forms upon admission.
Assist nursing as needed:
Print current PT/INR calendar is available for nurses. Prepare for provider rounds and give to nurse responsible for provider rounds. Print weight and blood glucose graphs, current physician orders, rounding Guide. Work as a team to assist in admissions, transfers, and discharge of residents. Holds responsibility to assist in provision of resident care when and where needed through flexibility in own schedule as deemed necessary by the DON. Report activities, problem areas, critical situations, resident complaints, physician complaints that are unable to be resolved or that are significant in nature to Administrator and DON. Has knowledge of and utilizes appropriate resident care protocol relating to the physical and psychological needs of adult and geriatric residents. Assist in the Materials Management department in procurement, inventory control, receiving and storage, and distribution of supplies for Luther Haven. Assists in maintenance of inventory, including a complete year-end physical inventory. Demonstrates knowledge of information systems in use for general departmental functions as well as those specific to job role. Maintains a clean and orderly environment. Adheres to the time and attendance policy. Reports violations of corporate compliance to supervisor when indicated. Other duties as assigned by Administrator or DON. Qualifications High school diploma or equivalent required. Training or experience in health information management, medical records, or office administration preferred. Experience in long‑term care or a healthcare environment is an asset. Knowledge of HIPAA and medical record confidentiality requirements. Strong attention to detail and accuracy in data entry. Proficiency with EHR systems and basic office software. Ability to handle multiple tasks, prioritize, and meet deadlines. Strong communication and customer‑service skills.
