Job Description
Job Duties:
Coordinate person-centered services for individuals with developmental and intellectual disabilities and autism in home and community settings. Serve as the primary contact for individuals, families, and Shared Living Providers (SLPs). Ensure services and care plans align with each person's unique needs and goals. Advocate for individuals and their families to secure appropriate supports and resources. Supervise direct support staff and provide guidance on service delivery. Deliver some direct care as needed. Manage essential administrative tasks related to service coordination. Collaborate with team members and community partners to support positive outcomes. Apply creative problem-solving to address challenges in service delivery.Qualifications Education and experience:
High school completion or equivalent (BA/BS in a relevant field of study preferred); Team leadership experience; and Three or more years' experience providing services to individuals with developmental and intellectual disabilities, including autism. Knowledge, skills, and competencies: Knowledge of evidence-based practices and supportive services for individuals with intellectual and developmental disabilities and autism, and familiarity with relevant community resources. Ability to collaborate and work productively with internal and community partners. Excellent organizational skills, with the ability to document work/services provided into an electronic records system with a high level of accuracy and timely completion. Crisis intervention and de-escalation skills, and the ability to appropriately and compassionately respond to client needs, while maintaining professional boundaries. Mission-driven approach to providing high-quality services to people with mental health concerns, substance use and co-occurring disorders, developmental and intellectual disabilities, and autism Problem-solving and critical thinking skills, with a solutions-oriented approach to addressing challenges. Ability to maintain a calm demeanor and to make sound decisions in challenging situations. Ability to drive, a valid driver's license, an acceptable driving record, and consistent, regular access to a safe, reliable, insured vehicle.
