Job Description

JOB SUMMARY
The Senior Administrative Assistant - Transitions of Care is responsible for independently coordinating and performing complex and specialized administrative and office duties to support Care Management and Social Services. These duties may include responsibility for tracking specialty patient populations, maintaining databases for continuity of care and follow up, compiling data and reports for quality review, department planning, acting as a liaison for internal projects, data maintenance and reporting, and coordination of projects, trainings, and meetings.
PRINCIPLE FUNCTIONS
includes the following, other duties may be assigned: Serves as system administrator for operations software including but not limited to work orders, badging and policy medical systems. Provides administrative support to the Transitions of Care and Social Work team as needed to support case management workflow process on a daily, weekly, and monthly basis. Maintains status of department accountabilities and/or requirements as assigned. Maintains meeting minutes and documentation as assigned. Acts as primary contact for administrative and/or specialized information regarding department accountabilities and/or projects. Acts as the primary department liaison for internal and external customers. Ensures department information is distributed and maintained with appropriate departments and external customers. Processes incoming internal / external mail as directed. Manages/coordinates the maintenance, review, and updating of department documentation, which may include contracts, orientation materials, standards, policies, and procedures, legal or corporate regulatory requirements. Other special projects related to quality improvement. Acts as a resource to department staff. Prepares letters, memos, spreadsheets, reports, and other documents as directed. Maintains office equipment as directed and contacts vendors for repair as needed.
JOB KNOWLEDGE & QUALIFICATIONS
Education:
Highest level of education completed, minimum high school diploma or equivalent required. Advanced college coursework in business preferred. Training and Experience A minimum of four (4) years' work experience, preferably in a health care setting, providing customer service and administrative support to professionals. Two (2) years' job-related experience including responsibility for coordinating complex administrative projects for management and associated professionals in an organization. Three (3) years MS Word and Excel software programs. Excellent interpersonal and communication skills.

$24.97 - $35.82
