Job Description

The Nuevo Dia Adult Day Health Center is a new and innovative program providing services to the Latino Community. We serve older adults, and people with disabilities with personalized hands-on healthcare and social services that will improve the quality of their lives. We thrive on delivering local and personal participant care in a culturally-competent manner. Our team is a fun, close knit group of people that believes in working together to help better our participant's lives and their health outcomes.
TASKS AND RESPONSIBILITIES
Building and tracking of key program checklists Put together and maintain employee files Prepare and schedule in-services Maintain participant attendance documentation including making sure that documents are up to date and include all required documentation Greet guests and provide them with superb customer service Ensure the front desk is neat, presentable, and equipped with all the necessary supplies such as pens, forms, and paper Answer all client questions and incoming calls Redirect phone calls to the appropriate department and take down messages Obtain all necessary paperwork for a potential client to be send to SCO, ASAP, etc Evaluate social and community needs of participants and families Interact with participants Maintain master list of clients Maintain list of Community Services available for seniors Assist participants with securing social and medical services through community agencies as needed as well as referring clients to available community services Document and follow up all community services received by the participants Assist participants in obtaining necessary medical services in the community not available through the day program (such as vision care, podiatry, medical equipment, etc.) Completes accurate and timely documentation of all services Assumes other job-related duties as assigned Facilitates assessment of general intake from potential clients or their referral sources Responsible for initial participant admission Collaborate with team members and referring professions to provide positive seamless services Provide assistance as detailed in interdisciplinary care plan Proactively identifies the needs of each participant and effectively provide services Effectively serves as the patient advocate in collaboration with patient's health care providers and community resources
QUALIFICATIONS
1.
Academic:
High School diploma or equivalent 2.
Experience:
Working in adult care setting 3.
Health:
Pre-employment clearance by a physician that certifies the employee to be free of infectious disease and/or restrictions that would inhibit performing job related responsibilities 4.
Language:
Able to read, write and speak English. Able to speak Spanish.
