Job Description

Patient Care Coordinator Silver Creek Family Medicine Silver Creek Family Medicine is seeking a friendly, organized, and patient-focused Patient Care Coordinator to join our front office team. This role is the first point of contact for our patients and families and plays a vital part in creating a welcoming, efficient, and compassionate clinic experience. Position Overview The Patient Care Coordinator is responsible for front desk operations including multi-line phone management, scheduling, insurance verification, patient registration, and financial collection. This position works closely with providers and clinical staff to ensure smooth patient flow and accurate documentation throughout the day. Key Responsibilities Patient Experience & Front Desk Operations Be at your workstation and prepared to answer phone calls promptly at 8:00 AM Greet and assist patients in person and over the phone in a friendly, confident, and professional manner Register and check in patients for scheduled appointments following clinic guidelines Assist with patient check-out and closing visit tasks Encourage and assist patients with MyHealth sign-up, password assistance, and proxy access Maintain a clean and organized lobby and reception area Coordinate with back office staff before leaving for the day Reception team may leave once the last patient has exited the building Scheduling & Communication Answer and appropriately route all incoming calls Schedule appointments accurately according to clinic protocols Document patient calls and route clinical messages to the appropriate team member Accurately document and submit new patient requests for provider review Utilize recall lists for Wellness Visits and Well Child Checks Send no-show letters as required Insurance & Financial Responsibilities Verify insurance eligibility and coverage, including researching accounts without real-time eligibility Confirm PCP assignment for patients with OHP Determine patient financial responsibility and collect co-pays at the time of service Scan insurance cards and ensure accurate insurance documentation in the patient chart Assist uninsured patients with payment expectations or payment plan setup Manage US Pay collections and automatic payment plans Administrative & Clinical Support Pre-register daily appointments and complete patient information prior to visits Sort prescription refill requests and route them appropriately Assist patients with completing required forms (sports physicals, DMV, DCL, etc.) Process medical records requests Receive and route faxed medical and specialist documents Provide professional support to providers, staff, patients, and visitors Perform basic technical troubleshooting related to front office systems End-of-Day Procedures Perform closing procedures Balance daily journals and submit to the billing department Pull and count cash accurately Professionalism & Self-Management Communicate with patients respectfully and confidently, including name and role introductions Maintain HIPAA compliance and adhere to clinic policies Treat patients with dignity, respect, and compassion Communicate openly and professionally with coworkers Collaborate with the care team to support excellent patient care Arrive to work on time and reliably Communicate clearly regarding breaks, lunches, and end-of-day coverage Follow up promptly on assigned tasks and patient inquiries Take pride in your work and strive for excellence
