Job Description
FUNCTION
Efficiently and professionally handles calls for Hospice of the Upstate and relays/directs messages and visitors to patient rooms while fulfilling additional administrative duties; provides a welcoming atmosphere to all who enter Hospice of the Upstate. Works with the weekend Admissions RN to assure a seamless flow of referrals toHOU. SUPERVISION RECEIVED

Serves as main switchboard operator for incoming phone calls in a high traffic area. Relays phone calls to appropriate staff or patient rooms. Responds to patient requests per intercom in a timely manner and directs staff to rooms as needed. Greets visitors and screens them according to HOU visitation guidelines. Communicates compassionately with patients and families. Accepts donations and completes appropriate forms. Assists other departments with tasks that can be completed at the front desk. Assists weekend Admissions RN in all aspects of HOU referrals, preparation and scheduling of appointments, collection of accurate contact information including patient address and expected presentation times. Receives calls from referral sources, provides timely response, and initiates paperwork to begin intake/referral process. Provides accurate and timely information and documentation regarding HOU services. Inputs HOU referrals into CareAnyware system within one hour of referral and completes first phase of admission process. Prepare information for Death Notifications (MDs, coroner, staff- document) Complete BRT's and coroner sheets for weekend deathsACCOUNTABILITY EXPECTATIONS
Attend monthly staff meetings. Attend Remembrance Services Communicate clearly with staff and supervisor. Ensures the protection of patient and employee information by followingHIPAA-HITECH
guidelines. Assures that employees maintain the integrity of all forms of patient information including but not limited to electronic personal health information. Assures that there are no attempts to access more than the minimum necessary information to complete his/her job. Be flexible, organized and work effectively and professionally under stress. Professionally represent the organization with credibility and dignity and in the spirit of excellence and trust. Demonstrate and communicate the mission, ethics, and goals of the facility as the face of Hospice.DESIRED MINIMUM QUALIFICATIONS
Education and Experience:
High School Diploma Must be of legal age to work in the state of South Carolina.Necessary Knowledge, Skills and Abilities:
Ability to multi-task, function independently and as a member of a team. Ability to maintain harmonious, constructive relationships with internal and external customers. Effective written and verbal communication skills to provide accurate and timely information to prospective patients, families, referral sources and the public. Demonstrates effective and rapid coordination of referrals from physicians and other referral sources. Demonstrate a working knowledge of Microsoft Word, Excel and general computer skills.TOOLS AND EQUIPMENT USED
