Job Description

Mount Pleasant Health and Rehab is seeking a compassionate and motivated Admissions Coordinator / Social Services professional to join our team! In this dual-role position, you will play a vital part in supporting both the admissions process and the social service needs of our residents and families. This is an excellent opportunity for someone who enjoys building relationships, providing support, and making a positive impact in a long-term care environment. As the Admissions Coordinator / Social Services team member, you will assist with coordinating admissions, communicating with families and referral sources, and helping ensure a smooth transition for new residents entering the facility. You will also provide emotional support, assist with resident needs and concerns, and work closely with interdisciplinary teams to help enhance the overall resident experience. Responsibilities:
Coordinate the admissions process for new residents, including communication with hospitals, referral sources, residents, and families Verify and collect required admission information, documentation, and insurance details prior to admission Assist with resident intake procedures and help ensure smooth transitions into the facility Conduct tours for prospective residents and families and provide information about facility services Support residents and families with social service needs, concerns, and adjustment to the facility environment Assist with discharge planning and coordination of services as needed Advocate for residents' rights and help address resident or family concerns appropriately Participate in care plan meetings and collaborate with the interdisciplinary team to support resident needs Maintain accurate documentation in accordance with facility policies and state/federal regulations Develop and maintain positive relationships with referral sources and community partners Assist with census development and occupancy goals Ensure confidentiality and professionalism in all resident interactions and documentation
Qualifications:
Previous experience in a long-term care setting preferred Prior experience in an Admissions, Marketing, or Social Services role preferred Demonstrated ability to understand and communicate clinical outcomes, services, and facility data effectively Experience networking and coordinating community outreach events preferred Strong communication and interpersonal skills required Ability to build and maintain professional relationships with referral sources, residents, and families Excellent organizational and multitasking abilities Bachelor's Degree required Social Services degree preferred Professional, compassionate, and customer service-oriented approach required
