LTCC RESUME REQUIRED THIS POSITION IS ONSITE & IS NOT ELIGIBLE FOR REMOTE WORK JOB SUMMARY
Serve as the Admissions/Discharge Coordinator for LTCC by performing all duties associated with the reception of inquiries through the coordination of admission and the discharge planning process.
JOB REQUIREMENTS
Bachelor's degree preferred. Hours may vary with occasional weekend and holiday shifts required. Will have to be flexible to conduct weekend admissions if they are required.
EXPERIENCE A
minimum of one year of previous experience preferred in a healthcare setting in a similar role preferred. SKILLS Must possess a high degree of confidentiality in all matters with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy. Must have strong interpersonal skills with the ability to communicate with staff and public in a friendly, courteous and helpful manner. Exhibits a special interest in the older adult, and others requiring long-term care. Must have excellent organizational skills with the ability to multi-task. Must have well-developed verbal and written communication skills. Strong computer and keyboarding skills. Must be proficient in the use of Microsoft Office Suite products knowledge including Word, Excel and Outlook. Ability to learn new computer software applications. Must have critical thinking skills. Accurate and systematical filing skills. Must know medical terminology. Time management skills are essential. Detail oriented. Exceptional phone etiquette.
ESSENTIAL FUNCTIONS
Receive and respond timely to all inquiry telephone calls, faxes, e-mails and on-line referrals, and distribute electronic copy of referral packets to members of referral team for review. Provide tours to inquiries and prospective residents Conduct in person visits of referrals, when possible, to conduct screening of needs. Collaborate with clinical representative and case manager(s) to ensure current, factual information is gathered regarding clinical, financial, behavioral, legal and other needs of the prospective resident as well as the prospective resident's anticipated length of stay (i.e., short-term rehab, long term, respite). Coordinate and direct referral team meetings to determine if prospective residents' needs can be met by the facility. Coordinate ongoing communications with referral sources to ensure all questions are answered, and all information is received to satisfy the ability to determine if needs can be met. Facilitate prior authorizations as appropriate for pending admissions when primary payer source requires a prior authorization. Ensure PASRR and LT101 process are completed in compliance with current regulatory requirements. Determine prospective admission date and time. Collaborate with direct care staff and IDT to determine room placement for new admissions. Respond to and facilitate all room changes in the facility including obtaining consents, providing notifications and documenting in the medical record. Facilitate notification to all staff when admission is scheduled along with room placement and information on ADL, clinical, and any known psychosocial needs of the incoming resident. Maintain and update referral database with clear, timely and accurate information Enter demographic and other pertinent information into PCC to initiate creation of electronic medical record for all new admissions prior to day of admit. Ensure resident's room is properly cleaned, prepared, and furnished according to the needs of the new admission (i.e., oxygen concentrator in place, bed placement based on physical needs of the resident, etc.). Complete admission packet with prospective resident or his/her representative prior to or upon admission. Complete admission and readmission assessments in the resident's medical record, including Social Determinants of Health and Trauma Informed Care. Complete MDS sections C, D, E and Q for all residents per MDS schedule to ensure timely transmission to CMS. Provide orientation to new admissions and their representative(s) of facility, staff, etc. Complete the discharge planning assessment in collaboration with the residents, family, IDT and provider for all residents who plan to discharge from the facility. Coordinate all referrals to community agencies as part of the discharge planning process. Document discharge plans and all updates throughout the discharge planning process in the medical record. Conduct discharge care conferences with the residents and/or family and IDT. Prepares and submits required PBJ data to CMS as required by regulation. Ensures that appropriate information regarding LTCC employees is sent to Human Resources in a timely manner. Manages and approves all LTCC employees' time sheets in the time and attendance system by deadlines set by accounting. Coordinates and provides orientation for student interns from local high school and colleges.
SECONDARY FUNCTIONS
Assist with answering phone and greeting visitors in the Business Office. May assist with Business Office functions such as cash transactions. Ensures census is accurate. Participate in IDT morning meetings, in-services, QAPI, and other meetings as appropriate. Work with CRH Marketing Department to assist with LTCC marketing.
