Job Description

An FNP or PA Family Care opportunity that blends purpose, balance, and growth awaits in a dynamic, patient-first primary care setting! 🌟 New grads are welcome!

This respected, independent rural health system is looking for an APC to serve their growing population and expanding efforts. Recognized regionally for quality, safety, and patient experience, the organization leverages modern technology such as a top-tier EHR, telehealth, and point-of-care ultrasound to elevate outcomes. Innovative population-health initiatives, behavioral health integration, and value-based care results demonstrate consistent performance in access and chronic disease management. Deep community support, philanthropic partnerships, and frequent accolades for service excellence make this an inspiring home for a clinician's career. For the Family Medicine Nurse Practitioner or Physician Assistant, that means strong leadership, dependable resources, and an environment where compassionate care thrives.

At a patient-centered rural family health clinic, the Advanced Practice Provider APP in Family Practice will practice broad-spectrum outpatient medicine across the lifespan. The clinic offers same-day access, preventive care, chronic disease management, women's health, and minor procedures consistent with training and privileging. Point-of-care testing, onsite lab partnerships, and embedded care coordination streamline visits and close care gaps. Integrated telemedicine and collaborative specialty outreach bring high-quality services close to home without the commute. For the Advanced Practice Provider (APP) in Family Practice, this means meaningful continuity and the chance to know multigenerational families.

Reporting to a Clinic Medical Director within Primary Care, the Advanced Practice Clinician (APC) in Primary Care collaborates closely with board-certified family physicians, practice leadership, and an interdisciplinary team. Support includes friendly nurses, medical assistants, care coordinators, pharmacists, behavioral health clinicians, and on-site administrative partners who ensure efficient clinic flow and patient access. Core responsibilities for the Advanced Practice Clinician (APC) in Primary Care include comprehensive evaluation, diagnosis, and treatment, preventive wellness, acute care visits, chronic disease optimization, and evidence-based care planning documented in a robust EHR. Depending on training and interest, scope may include procedures such as joint injections, skin biopsies, and women's health services, with mentorship readily available. Light call, protected administrative time, and quality resources help the Advanced Practice Clinician APC in Primary Care stay focused on outcomes and relationship-centered medicine.

The Family Practice Advanced Clinician (NP/PA) will appreciate a region that blends small-town charm with big-time adventure, featuring nearby mountains, winding rivers, and endless trails. Weekends bring hiking, mountain biking, kayaking, skiing, and fly-fishing under bluebird skies 🌲. A lively calendar of concerts, festivals, farmers markets, and community events keeps the culture scene vibrant 🎢. Cost of living stays friendly, commutes are easy, and a four-season climate invites an active, wellness-focused lifestyle with farm-to-table dining, craft coffee, and local breweries. Excellent schools, welcoming neighborhoods, and abundant family activities make it an ideal home base for the Family Practice Advanced Clinician (NP/PA).

The Advanced Practice Clinician will receive competitive base salary, plus RVU initiatives, along with a full suite of benefits and a flexible customizable schedule. Connect with a CMC recruiter today to discuss this amazing opportunity!
