Job Description

Overview We are seeking a dynamic and highly skilled Advanced Practice Provider (APP) to join our healthcare team. This vital role involves delivering comprehensive patient care across a variety of clinical settings, utilizing advanced clinical skills in acute and chronic care management collaborating closely with physicians, nurses, and other healthcare professionals. The ideal candidate will bring a proactive approach to patient assessment, diagnosis, treatment planning, and follow-up care, ensuring exceptional health outcomes. Duties Conduct thorough patient assessments, physical examinations, and detailed medical histories to inform diagnosis and treatment plans. Manage acute and chronic conditions across diverse specialties Perform procedures including suturing, joint injections, and mild office procedures. Utilize electronic health record (EHR) systems such as Epic and Cerner for accurate documentation of patient encounters, medication administration, and clinical notes. Coordinate diagnostic evaluations including laboratory tests (phlebotomy), medical imaging, and interpret ICD 10 coding for accurate documentation. Provide patient education on disease management, medication adherence, and lifestyle modifications. Utilize EMR system for comprehensive medical records management. Requirements Valid certification as a Nurse Practitioner (NP), Physician Assistant (PA), with current state licensure. Proficiency with EMR/EHR systems. Strong knowledge of physiology, anatomy, and pharmacology. Familiarity with protocols including HIPAA compliance and OSHA. Ability to work effectively in outpatient setting with diverse patient populations including geriatrics patients.
Pay:
$85,000.00 - $90,000.00 per year
