Job Description Family Medicine APP Full-time West Valley Clinic - Intermountain Health West Valley City, UT Intermountain Health is an integrated, not-for-profit health system with hospitals and clinics throughout the Intermountain West. Incentives:

Clinic Hours:

Monday - Friday, 7:30AM - 5:00PM/36 patient facing hours; Tuesdays, late Clinic is offered as an option.

Minimum Qualifications:

Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN). Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement). Eligible for State Controlled Substance License II through V. Eligible for Federal DEA Certificate II through V. BLS Certification for Healthcare Providers. Other certification as required (e.g.

ACLS, PALS, TNCC, NRP

).

Preferred Qualifications:

Two (2) or more years of experience preferred. Bilingual (English/Spanish) preferred. Master's degree. Education must be obtained from an accredited institution. Degree will be verified.
