Job Description
UVA Health is seeking a highly skilled and motivated Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join the newly established Healthy Aging Clinic within the Department of Medicine's Section of Geriatric Medicine. This is a opportunity to serve as clinical team member in a newly established, multidisciplinary clinic purpose-built to address the comprehensive health needs of aging adults. The successful candidate will provide high-quality, evidence-based, patient-centered care to geriatric populations in both independent and collaborative practice models. Following an orientation and ramp-up period, the APP will maintain a schedule of eight 4-hour patient-facing clinical sessions per week, complemented by 8 hours dedicated to non-clinical and administrative responsibilities. This position is affiliated with the UVA Health Center for Advanced Practice (CAP), the system-level home for advanced practice governance, professional development, and provider support. Through CAP, the APP will have access to a robust infrastructure for APP career growth, peer connection, and operational guidance.
Preferred Qualifications:
- 1-3 years of clinical experience in geriatrics, internal medicine, or a related specialty
- Experience in or strong interest in multidisciplinary team-based care models
- Familiarity with comprehensive geriatric assessment, cognitive screening, and frailty evaluation
- Experience with Epic or comparable electronic health record systems
- Certification in Geriatric Medicine (AGPCNP-BC, GNP-BC, CAQ-Geriatric Medicine, or equivalent) Background checks and pre-employment health screenings will be conducted on all new hires prior to employment.
Clinical:
- Conduct thorough history-taking, physical examinations, and comprehensive geriatric assessments including cognitive, functional, fall-risk, and frailty screening
- Diagnose and manage acute, chronic, and complex medical conditions common to aging populations, including but not limited to dementia, polypharmacy, osteoporosis, delirium, and falls
- Order, interpret, and act on diagnostic studies including laboratory, imaging, and specialist consultations in a timely and clinically appropriate manner
- Develop, implement, and regularly reassess individualized, patient-centered care plans in collaboration with physicians, nursing staff, social work, pharmacy, and other members of the multidisciplinary team
- Perform appropriate clinical procedures within scope of practice and privileges
- Manage transitions of care effectively, coordinating with inpatient teams, post-acute facilities, home health, and primary care providers as needed
- Adhere to evidence-based clinical guidelines and UVA Health protocols while integrating patient goals, preferences, and values into care decisions
- Maintain documentation standards in the electronic health record consistent with UVA Health policies, regulatory requirements, and billing compliance
- Participate in on-call or cross-coverage responsibilities as determined by the clinical team and division leadership
Administrative:
- Maintain accurate, timely, and compliant clinical documentation including progress notes, after-visit summaries, referrals, and prior authorizations
- Complete billing and coding in alignment with UVA Health billing standards and APP-specific expectations
- Respond to patient messages, prescription refill requests, and inter-provider communications within expected turnaround times
- Utilize the 8 dedicated weekly non-clinical hours for administrative responsibilities including documentation completion, care coordination, inbox management, and team communication
- Adhere to UVA Health policies and procedures related to credentialing, privileging, licensure renewal, and compliance training requirements
- Contribute to quality reporting and population health data collection as required by clinic and system-level programs
- Engage in performance improvement activities, including review of outcome data and participation in root cause analyses or case reviews as indicated
Leadership:
- Serve as a visible and invested contributor to the development of clinical workflows, care protocols, and operational infrastructure
- Collaborate with physician leads, clinic administration, and other leaders to build a patient-centered, team-based practice culture reflective of UVA Health's mission and values
- Mentor students, trainees, and newly onboarded APPs as appropriate, while modeling clinical excellence and professional standards
- Participate in team meetings, case conferences, and huddles, contributing to team-based decision-making
- Serve as a liaison between frontline clinical staff and APP/administrative leadership on issues related to workflow, patient safety, or practice concerns
- Advocated for initiatives that improve patient experience, clinical quality, and operational efficiency
Professional:
- Maintain all required licensure, certifications, and credentialing in good standing and in compliance with applicable state, federal, and institutional requirements
- Engage in continuing medical education and professional development activities to sustain and expand clinical competency in geriatric medicine and advanced practice
- Engage actively with CAP, including participation in APP-specific forums, advisory groups, or committees as able
MINIMUM REQUIREMENTS
Education:
Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree.Experience:
Department and unit specific.Licensure:
License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position.PHYSICAL DEMANDS
