Our team of experienced Advanced Practice Registered Nurses (APRNs) and healthcare professionals strives to improve patient outcomes and enhance quality of life through accessible, comprehensive care. Overview We are seeking a highly motivated and skilled Certified Nurse Practitioner to join our team. In this role, you will provide expert clinical care across diverse settings, including Memory Cares, ALF, SNF, and home patients. You will play a vital role in delivering high-quality healthcare tailored to individual patient needs while working within a collaborative, patient-centered environment. Duties Conduct thorough patient assessments, including physical examinations and vital signs, to determine health status and develop personalized care plans. Perform diagnostic evaluations utilizing laboratory testing, medical imaging, and other diagnostic tools; interpret results accurately. Engage in telehealth consultations and triage to deliver timely advice and interventions for acute pain management and urgent care needs. Skills Proven experience with EMR systems and Athenahealth or similar EHR platforms for efficient documentation. Strong background in diverse clinical settings. Familiarity with CPT coding (Current Procedural Terminology), ICD-10/ICD-9 coding systems for accurate billing and documentation. Knowledge of infection control protocols and aseptic techniques essential for safe practice across outpatient and inpatient environments. Ability to perform advanced assessments involving physiology knowledge and diagnostic evaluation techniques. Experience working with diverse populations including children (pediatrics), seniors (geriatrics), patients with dementia or Alzheimer's disease (memory care), or those requiring post-acute or home care services. Skills in managing complex cases involving acute pain management, occupational health issues like Workers' compensation law compliance, or infectious disease care. Excellent communication skills for health coaching, patient education on medication administration/injection techniques, discharge instructions, and family planning discussions.
