Job Description
Job Summary We are seeking an experienced Nurse Practitioner or Physician Assistant to join our physician-led medical group providing care to patients in skilled nursing, post-acute, and long-term care facilities throughout the Tulsa area. This is a 1099 independent contractor position offering flexible scheduling, strong physician support, and the opportunity to practice with a high level of autonomy. Our providers work within a collaborative, team-based model of care, partnering closely with physicians, nursing staff, therapists, and facility leadership to ensure the highest quality outcomes for our patients. - Established relationships with skilled nursing and assisted living facilities
- Administrative and billing support provided by the practice
- Opportunity to grow with an expanding post-acute and long-term care medical group Duties
- Provide primary care services to patients in skilled nursing and assisted living facilities
- Perform routine follow-up visits, acute visits, and transitional care visits
- Collaborate with physicians, nursing staff, and facility leadership
- Manage chronic medical conditions common in geriatric populations
- Provide patient and family education regarding treatment plans
- Maintain accurate documentation within the EMR Requirements
- Licensed Nurse Practitioner OR Licensed Physician Assistant in the State of Oklahoma
- Board Certified (FNP, AGNP, or Adult NP; or PA-C)
- DEA license (or ability to obtain)
- Experience in long-term care, skilled nursing, or geriatric medicine strongly preferred
- Strong clinical decision-making and communication skills
- Ability to work independently while collaborating with supervising physicians
Pay:
$115,000.00 - $150,000.00 per yearWork Location:
