Home Coordinator

Primary Function:

Assists with the activities of all CILA homes, both continuous care and intermittent care, with the Manager of Community Living Services. Responsibilities include maintaining and monitoring all consumer files, while conducting programs and collecting data in accordance with the consumer's individual rehabilitation program plans for the CILA program. Assists with medication monitoring, safety procedures and drills, community activities, meal planning, and grocery expenditure, while assisting in all aspects of activities of daily living and encouraging the CILA consumer to maintain an independent life style, providing transportation services as needed. Assists with scheduled appointments and ensures timely completion of all Dental and Medical Appointments as needed to ensure the health and well-being of all individuals as well as ensuring the requirements set forth in Rules 115 and 116 are met. Required to attend all institute days, and staff and consumer meetings as requested by the Residential Service Manager. Shares in the on-call duties in coordination with the Residential Services Manager/QIDP/other applicable staff. Ensures that the Department of Mental Health and Developmental Disabilities Rule 115 is interpreted and applied correctly for all CILA consumers. As a member of the MEE staff, the Home Manager is also responsible for protecting and advocating for consumer rights. All positions require a criminal background check. Hours are 6am-2:30pm Must have a valid Illinois driver's license and reliable transportation
