Position Description:
Functions as the TNC Community Disability Professional Manager for the Group Homes and Individual Supported Living (ISL) locations to facilitate the programmatic process dealing with habilitation service delivery. Responsible for coordinating staff program orientation and the maintenance of agency supports as outlined in the Individual Support Plan. Functions as the designee for the advocacy program for persons served and supportive operations. Monitors support delivery and related documentation regarding site-based records. The QDDP will advocate for the removal of architectural, environmental, attitudinal, and financial, employment, communication, transportation, community integration, and any other barriers to the persons receiving services through TNC Community. Responsible to report all suspected/alleged abuse and neglect in accordance with TNC Community policies. TNC Community is an equal opportunity employer. PROGRAM RESPONSIBILITY FOR PERSONS SERVED/ESSENTIAL FUNCTIONS
1. Oversight/coordination of all of the individual's programs and support services being received. 2. Charged with direct plan implementation, quality enhancement monitoring, establishing information adjustment, documentation and training of Individual Support Plans (ISPs). 3. Facilitate and coordinate annual and additional Individual Support Plan meetings. Submit ISP information to appropriate Support Coordinator according to guidelines. 4. Attend service coordination and parent/guardian meetings. 5. Post to server annual ISP schedule, indicating month meeting is due and date of implementation. Schedule ISP meetings at least 30 days prior to month due and update ISP schedule on server. 6. Submit all ISP related documents according to ISP schedule. 7. Establish information collection systems. 8. Direct and monitor plan implementation for individuals as needed. 9. Monitor the protection of individuals' rights and confidentiality issues. Hold quarterly individual rights meeting and document. 10. Produce monthly reviews for individuals according to requirements and forward to appropriate Support Coordinator and/or post to server according to guidelines by the 15th of each month. 11. Maintain individual records related to programming. 12. Correspond with referring agencies and any other service provider for program related issues. 13. Be familiar with CARF Standards and DMH regulations and develop and implement training services related to programmatic processes within those guidelines. 14. Participate in development of policies and procedures, particularly as related to programmatic processes. 15. Maintain logs/documentation as required by Medicaid Waiver.
HEALTH/SAFETY RESPONSIBILITIES/ESSENTIAL FUNCTIONS
1. During onsite location visits and direct activities; observe individual interactions, and monitor supportive equipment for safety, hygiene/cleanliness, and appropriate function. Report and implement necessary actions and repairs as needed. 2. Regularly review Event and Medication Error reports for necessary response/actions. 3. Regularly monitor health care tracking data and Community RN reports to ensure that all appointments, labs, consults and results are scheduled, maintained and documented. 4. Complete a minimum of one (1) site visit between the hours of 12am and 5am every month at assigned sites for the purpose of verifying compliance and performance quality by PSA staff. 5. Report suspected/alleged abuse and neglect per TNC Community policies and procedures. 6. Respond and assist those supervised to respond to Health & Safety Self-Inspection and External Inspection needs.
PERSONNEL RESPONSIBILITIES/ESSENTIAL FUNCTIONS
1. Responsible for new staff orientation/training related to programmatic processes. 2. Responsible for ongoing staff training and monitoring related to programmatic processes. 3. Support Residential Support Supervisors, Lead Personal Support Attendants (PSA) and Personal Support Attendants (PSA) assigned to supervised sites by serving as a mentor, assisting in the training process for individuals receiving services and personnel and by providing oversight for specified tasks. 4. On-call responsibilities as deemed necessary. This may include being available for 24-7 consultation by on-call staff. May be required on occasion to provide direct care staffing (completing PSA duties) onsite.
FINANCIAL RESPONSIBILITIES/ESSENTIAL FUNCTIONS
1. Oversee monthly/quarterly/annual budgets of supervised sites by regular meetings with RSS and documentation review. 2. Responsible for monitoring personal spending accounts of individuals for use, documentation and receipt submission in a timely manner. 3. Responsible for monitoring petty cash expenditures for supervised sites, documentation, and receipt submission in a timely manner. 4. Responsible for corporate credit card for necessary expenditures. Will provide receipts for all expenditures and submit to finance in a timely manner.
FACILITY/TRANSPORTATION RESPONSIBILITIES/ESSENTIAL FUNCTIONS
1. Maintains TNC Community vehicles in an organized, clean, safe manner. 2. Ensures safe and continuous operation of TNC Community vehicle by reporting repair needs, accidents, and traffic violations immediately. 3. Monitor facilities for cleanliness, safety issues and functionality for the individuals supported
OTHER RESPOSIBILITIES
1. Participate in scheduled staff/house meetings, providing training, information, etc. as indicated. 2. Participate and/or chair a variety of committees as needed to support programmatic processes. 3. Submit all required reports according to guidelines. 4. Follows all TNC Community rules, policies, and procedures 5. Other duties as assigned.
EDUCATION AND EXPERIENCE QUALIFICATIONS
Meets the qualifications for Disability Professional Management per the Missouri Medicaid Waiver rules. Bachelor's degree preferred. Two years of combined direct service and related management experience preferred (must have at least one year of direct service experience).
COMMUNICATION
The position requires sufficient communication skills: written, verbal, and nonverbal, to effectively engage with individuals, staff and administration.
ADDITIONAL REQUIREMENTS
Ability to clear background checks and drug/alcohol screenings as required. TNC Community's drug and alcohol testing programs comply with the Federal Transit Administration regulation 40 CFR Part 655. Current CPR/First Aid certification Maintain current Abuse/Neglect Training Completion of Gentle Teaching or Equivalent Must participate in all mandatory trainings and re-certifications as required. Ability to drive motorized vehicles Valid class E Missouri driver's license or equivalent Must have own transportation and proof of insurance Ability to lift 75 pounds (weight may vary) Flexible schedule Be familiar with and able to utilize a computer
PHYSICAL EXPECTATIONS
These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the QDDP's position. He/she lifts and moves items and/or persons up to 75 lbs., and greater weights utilizing a lift device or a two person lift/transfer. Visual ability required by this position includes close vision.
