Job Description

PennReach is a non-profit who provides full service/in home care to individuals with special needs. We support individuals to live in apartments of their own in various affordable housing developments, as well as in our group homes in the state of New Jersey. Our goal is to not only provide individuals with housing, but to maintain the supports to give our residents the life they deserve PennReach is currently looking for a coordinator to oversee 2 suites with 8 individuals. This is a medical location and will require oversite of LPN's also.
SUPERVISES
Direct Support Professionals (DSPS), Lead DSPs and LPNs (as assigned), Managers, Assistant Managers, Care Coordinators.
PURPOSE:
To use best practices and technology within a community based residential setting to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible.
REQUIREMENTS
Bachelor's degree or High school diploma or equivalent with 4 years of experience working with people with developmental disabilities Driver's license valid in the State of NJ with good driving record Shall be at least 18 years of age or older Employee must cooperate with the licensee and Department staff in any inspection or investigation.
RESPONSIBILITIES
1. Ensures full staffing on all shifts. Covers shifts as needed to ensure full staffing ratios. 2. Must successfully complete and demonstrate proficiency in all areas of required training. 3. "On call" 24 hours a day in cooperation with other management staff. 4. Objectively completes staff evaluations. 5. Monitors and ensures that residential budget is maintained. 6. Monitors and schedules required staffing schedules, staff training, as well as relevant seminars, conferences and additional training. 7. Monitors and insures accurate "in house" documentation. 8. Participates in the Individual Support Plan (ISP) for the individuals served. 9. Monitors apartments and vehicles maintenance and arranges for repairs as appropriate. 10. Ensures the accuracy of the disbursement and documentation of individual finances, petty cash, gas cards, Shoprite cards as needed. 11. Monitors health and medical needs of individuals and audits Medication Administration, Critical Logs/CareLogic entries, BSP charts, and specialized charting for individuals served (BM charts, for example) 12. Conducts staff meetings/trainings as appropriate. 13. Ensures that all staff on shift are fully trained in all required trainings, including mandatory trainings for all staff and all specialized trainings for each home. 14. Monitors the documentation of each individual's progress and provides documentation as requested to for involved parties including family/guardians, support coordinators, day programs, employment specialists, case manager and other professionals, as necessary.15. Ensures consistent service delivery which connects work, home, and recreational needs. 16. Submit monthly written reports covering operational status, as well as individual progress and concerns to supervisor. 17. Attend conferences, seminars, and training relevant to the position. 18. Coordinates IDT meetings & the ISP, serving as an active member of each person's interdisciplinary team, to include involvement in Individual Support Plan development. 19. Participates in all aspects of the individuals supported life and care. This includes but is not limited to cooking, cleaning, transporting, assisting with hygiene, etc. 20. Conducts random site visits to ensure the quality of the programs supervised. 21. Perform any and all duties requested of supervisor.
INITIATIVE REQUIRED
Fulfillment of the job responsibilities takes place in the residence, agency, and in the community. Work is reviewed by the supervisor according to the policies of PennReach, Inc. This review includes such things as random site visits, systematic assessments, and employee evaluations. The Care Coordinator must be able to handle the pressure of training in a public environment, exercise patience, work with flexible schedules, and respond to crisis situations. The ability to effectively communicate with subordinate staff, other agencies and members of the community is crucial to successful integration. The position also requires the full use of good judgment and common sense during daily affairs. Professional communication and appropriate language, whether written or oral, is essential in everyday activities. This communication may be with any person involved in the life of the individuals living in the home. Ingenuity and creativity must be utilized when determining the best training methods, assistance and supports for an individual.
