Lifesharing Program enables individuals with developmental disabilities to live in a family home environment, share life experiences and pursue opportunities with supportive people. Lifesharing allows individuals to live to their fullest potential by becoming active, participating members of another family. As a Coordinator for the Lifesharing Program, you will work carefully to match the right person with the right family or person. The essential functions of this position include:

  • Maintain client and providers files in accordance within
DPW 6500
regulations
  • Preparing sites prior to licensing or moving a client into a home
  • Participate in the interview and selection process, orientation, evaluation and supervision of Lifesharing Providers
  • Coordinate special client activities such as the Christmas brunch and bi-monthly evening client activities
  • Track trainings for Lifesharing providers and ensure each provider meets the state requirement of 24 hours training per year
  • Organize and conduct quarterly Provider trainings
  • Conduct Medication Administration trainings for Lifesharing providers and

staff and do annual practicums
  • Keep track of Lifesharing incidents and attend monthly Incident Management meetings Qualified candidates will have:
  • Bachelor's Degree in related field including social work, psychology, special education or human services.
  • Three (3) years of experience working as a coordinator for individuals with developmental disabilities.
  • Two (2) years of direct care experience with individuals with developmental disabilities.
  • Current certifications in First Aid, CPR and medication administration.
  • Computer proficiency in MS Office including Word, Outlook and Google Docs.
  • Proficient in HCSIS, the DPW incident reporting system and the agency data system.
  • Valid driver's license All candidates must be able to secure PA criminal, FBI and child abuse clearances.
