Hospital Social Worker - Up to $130K/YR! + $5,000 Sign on bonus!

Provide comprehensive social work services to patients and families within a hospital setting, collaborating with a multidisciplinary team to facilitate smooth care transitions and support emotional and practical needs.

Key Responsibilities:

Assess patients' social, emotional, and psychological needs through individual and family interactions. Provide counseling and support services to patients and families facing medical challenges. Develop discharge plans in collaboration with healthcare providers to ensure smooth transitions. Offer resources and referrals to community services, including housing, financial aid, and mental health support. Assist with crisis intervention, helping patients and families cope with urgent or complex healthcare situations. Advocate for patients' rights and assist with navigating the healthcare system. Document all interactions and interventions in the medical record per hospital policies. Participate in care planning meetings and case reviews with medical staff. Ensure compliance with all applicable laws, regulations, and hospital policies.

Qualifications:

Bachelor's degree in Social Work required. Licensed Clinical Social Worker (LCSW) required. Valid California state licensure as a Clinical Social Worker required.

Preferred Skills:

Experience in medical social work, hospital-based settings, or healthcare environments. Strong knowledge of community resources and services in the local area. Experience with crisis intervention and working in a multidisciplinary team. Ability to work independently and manage multiple cases simultaneously. Minimum of two years of clinical experience in a hospital setting. Basic Life Support (BLS) certification.
