Job Description

Overview Join our dynamic healthcare team as a Licensed Master Social Worker (LMSW) and make a meaningful difference in patients' lives! You will be at the forefront of delivering compassionate mental health counseling, conducting thorough patient assessments, and coordinating care plans that enhance well-being. Responsibilities Conduct detailed patient assessments to identify social, emotional, and environmental needs, ensuring holistic care planning. Provide individual, group, and family counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy. Collaborate with multidisciplinary teams to develop and implement personalized treatment plans aligned with clinical research and best practices. Manage case documentation accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations to protect patient confidentiality. Facilitate crisis intervention and manage urgent situations with professionalism and sensitivity, especially when working with individuals experiencing trauma or PTSD. Coordinate discharge planning and post-acute care services for patients with complex needs such as dementia, Alzheimer's disease, or developmental disabilities. Engage in ongoing professional development by staying current on medical terminology, ICD coding (ICD-9/10), CPT coding, and other relevant clinical documentation standards. Skills Proven experience in child & family counseling, hospice care, geriatrics, pediatrics, or working with individuals with disabilities. Strong knowledge of mental health counseling techniques including behavioral health interventions and diagnostic evaluation methods. Familiarity with EMR systems, ICD coding standards, CPT coding, and medical records management. Excellent communication skills for conducting intake interviews, clinical counseling sessions, and group therapy. Ability to perform crisis management effectively while demonstrating cultural competence and empathy. Experience working within hospital settings or nursing homes is highly desirable.
