Social Work Designee - Methodist Fremont Dunklau Gardens - Full Time

Nebraska Methodist Fremont Hospital is seeking a dedicated Social Work Designee to work at Dunklau Gardens . This is a full-time, day shift position. The social work designee supports the resident, family, physician and the interdisciplinary team in facilitating patient care with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction.

Job Duties & Responsibilities:

Develop and coordinate comprehensive discharge plans, identifying barriers early and arranging appropriate post-acute services to support safe, sustainable transitions of care. Collaborate with patients, families, and the interdisciplinary healthcare team to create and implement individualized care plans, participating in care conferences, rounds, and family meetings. Provide psychosocial assessments, counseling, and advocacy for patients and families, addressing complex social, behavioral, financial, and healthcare-related needs. Facilitate advance care planning discussions, including advance directives, power of attorney documentation, living wills, and end-of-life care preferences. Maintain accurate and timely documentation in the medical record and MDS, ensuring compliance with regulatory requirements and effective communication among the care team. Promote quality patient outcomes through community engagement, resource coordination, culturally sensitive care, and adherence to organizational safety and patient communication standards.

Salary:

Pay starts at $20.39 an hour and may increase based on work history/experience.

Job Requirements:

Bachelor's degree in social work required. May consider a Bachelor's degree in public health, human service counseling or social services. Working towards licensure as a licensed social worker (LSW) preferred. Master's degree (e.g., MA, MS, MBA, MSN, MSW) or equivalent (e.g., CPA, PE, etc.) preferred. Master of Social Work (MSW) degree from a school accredited by the Council on Social Work Education preferred. Current American Heart Association Basic Life Support (BLS) strongly preferred to have at time of hire, required within 3 months of hire. 3 years of experience as a social worker designee or like experience working with the geriatric population required. 2-3 years of experience as a medical social worker preferred. Experience working with the geriatric population within the healthcare environment preferred.
