Olympic Community Action Partnership is seeking an ARP - Homelessness to Housed Case Manager in Port Townsend, Washington. This is a full-time position with benefits. TheHOME-ARP
Case Manager / Housing Navigator provides intensive, mobile, housing-focused case management to individuals and families served through OlyCAP's Homelessness to Housed Intensive Case Management project. The position supports allHOME-ARP
qualifying populations, including people experiencing homelessness, households at risk of homelessness, survivors of domestic violence and human trafficking, and other highly vulnerable households for whom supportive services are necessary to prevent homelessness or housing instability. This position serves as a primary point of contact for participants and provides coordinated support across housing, healthcare, behavioral health, benefits access, legal services, and other stabilization needs. The Case Manager works in field-based settings throughout rural communities, including shelters, encampments, vehicles, community locations, offices, and scattered-site housing. The position uses a person-centered, trauma-informed, progressive-engagement approach that emphasizes participant choice, housing stability, health access, and long-term self-sufficiency.ESSENTIAL JOB FUNCTIONS
Conduct outreach, engagement, screening, and intake for eligible participants across all fourHOME-ARP
qualifying populations. Complete eligibility determinations and maintain required documentation in accordance with HOME-ARP, HUD, Commerce, and agency requirements. Develop individualized service plans with participants using a person-centered, strengths-based, and trauma-informed approach. Provide ongoing case management, including regular participant contact, reassessment, service coordination, advocacy, and progress monitoring. Assist participants in locating, applying for, obtaining, and maintaining permanent housing. Support participants with housing search, landlord engagement, rental applications, lease-up steps, tenancy support, and housing stabilization. Coordinate and facilitate access to healthcare, behavioral health, outpatient treatment, benefits, education, employment resources, childcare, food supports, transportation, and legal services. Provide direct assistance with obtaining critical documents such as identification, birth certificates, benefit verification, and other records needed for housing or services. Administer or coordinate eligible financial assistance and supportive services consistent with project guidelines and funding rules. Provide transportation support or coordinate transportation needed for housing access, healthcare, behavioral health, and other essential services. Meet participants in field-based and nontraditional settings, including encampments, vehicles, shelters, community centers, and scattered-site housing. Maintain accurate, timely, and complete case records, service documentation, financial assistance records, and outcome tracking in CAP60, HMIS, and other required systems. Coordinate with community partners, including medical providers, behavioral health agencies, victim service organizations, community paramedics, public health, shelters, landlords, legal providers, and other referral partners. Participate in case conferencing, partner coordination meetings, staff meetings, and required training activities. Apply confidentiality and safety protocols, including heightened privacy protections for survivors of domestic violence, sexual assault, stalking, and human trafficking. Monitor participant progress, adjust service plans as needed, and support participants through crisis, stabilization, housing placement, and follow-up. Help prevent duplication of benefits by coordinating services across systems and verifying existing supports. Contribute to program reporting, quality improvement, and continuous refinement of rural service delivery strategies.CORE COMPETENCIES
Strong knowledge of housing-focused case management, homelessness response systems, and supportive services coordination Ability to work independently in rural and field-based settings Strong communication, documentation, organization, and problem-solving skills Ability to build trust with participants experiencing crisis, trauma, behavioral health needs, or housing instability Ability to collaborate effectively with multiple partner systems and community providers Commitment to person-centered practice, equity, confidentiality, and professional boundaries Ability to manage competing demands while maintaining compliance and quality documentationMINIMUM QUALIFICATIONS
Bachelor's degree in social work, human services, psychology, public health, or a related field; relevant lived or professional experience may be considered in place of formal education where appropriate Experience providing case management, housing navigation, outreach, healthcare navigation, victim advocacy, or other direct services to vulnerable populations Experience working with people experiencing homelessness, behavioral health challenges, trauma, domestic violence, substance use disorders, or complex barriers to stability Knowledge of local community resources and service systems, or ability to develop this knowledge quickly Valid driver's license, reliable transportation, and ability to travel regularly throughout rural service areas Ability to use data systems, case records, spreadsheets, email, and standard office software for documentation and reportingPREFERRED QUALIFICATIONS
Experience with HOME-ARP, HUD, ESG, CoC, or other federally or state-funded housing and supportive services programs Familiarity with HMIS, CAP60, or other comparable data systems Training or demonstrated experience in trauma-informed care, motivational interviewing, Housing First, harm reduction, Mental Health First Aid, or survivor-centered advocacy Knowledge of landlord-tenant issues, housing barriers, and benefits systems Experience working in rural communities and field-based service delivery modelsWORK ENVIRONMENT AND SCHEDULE
This position is primarily mobile and field-based. Work will occur in offices, participant homes, shelters, vehicles, outdoor locations, community settings, healthcare environments, and other service sites throughout Jefferson and Clallam Counties. The schedule is generally business hours, with flexibility required to respond to participant needs, transportation barriers, urgent situations, and outreach opportunities.SUPERVISION AND PERFORMANCE EXPECTATIONS
This position reports to OlyCAP's Director of Housing and Community Development or designee. Performance will be measured by quality of participant engagement, timeliness and accuracy of documentation, housing and service outcomes, compliance with grant requirements, effective coordination with partners, and contribution to equitable access across allHOME-ARP
