Job Description:

Clinical Care Coordinator Strengthen your leadership and clinical decision-making skills by overseeing nursing operations, supervising staff performance, maintaining adequate staffing levels, and ensuring quality care and safety standards are consistently met for all individuals served. Expand your healthcare coordination and communication experience by collaborating with physicians, interdisciplinary teams, families, guardians, and service providers to ensure accurate assessments, individualized care planning, medication management, and continuity of care. Enhance your training, mentoring, and compliance knowledge by conducting staff trainings, monitoring policy adherence, participating in audits and corrective actions, and ensuring all safety procedures, abuse reporting requirements, and regulatory standards are properly followed. Develop your assessment, documentation, and problem-solving abilities by completing comprehensive health assessments, reviewing physician orders, monitoring responses to treatments, maintaining accurate reports and records, and implementing appropriate nursing interventions through resolution. Build your operational and supervisory expertise by assisting with admissions and discharges, supporting employee health needs, participating in administrator on-duty responsibilities, and serving as Acting Health Services Director when needed.

Job Requirements:

Use your compassion, leadership, and clinical knowledge to support individuals and improve the quality of care provided every day. Apply your nursing license (RN preferred) while supervising staff, coordinating healthcare services, and ensuring policies and procedures are followed. Brag about your ability to pass a Federal Background investigation. Improve your driving skills by presenting a Valid Drivers license.
