As a Home Health Aide/Personal Care Aide, you will provide personalized care and support to our residents, enabling them to live a fulfilling life in our warm and vibrant community.

• Responsibilities:

• Assist residents with personal care activities, such as bathing, dressing, and grooming, while promoting independence and dignity
• Provide assistance with mobility, transfers, and ambulation, as needed
• Help residents with daily living tasks, including meal preparation, medication management, and household chores
• Engage residents in activities and programs, such as exercise classes, arts and crafts, and social events, to promote cognitive and emotional well-being
• Develop and maintain positive relationships with residents, families, and staff members
• Participate in care planning meetings to ensure individualized care and support
• Maintain accurate and timely documentation of resident care and activities
