You'll round regularly, lead multidisciplinary care, and use decisive, evidence-based interventions to improve outcomes and reduce avoidable hospitalizations. Key Responsibilities Provide direct, comprehensive medical care to residents in long-term, subacute/post-acute, ventilator, and behavioral health settings. Conduct routine rounding, thorough evaluations, and timely follow-ups to ensure continuity of care and early, aggressive intervention. Lead and actively participate in multidisciplinary care team meetings; contribute to individualized care plans. Aggressively manage chronic and complex conditions; respond promptly to status changes and emergent situations using appropriate modalities (e.g., IV diuretics, antibiotics, fluids, TPN) and respiratory therapies. Communicate clearly and compassionately with nursing staff, NPs/APPs, patients, families, and consulting providers. Collaborate with facility nursing, rehabilitation teams, leadership, and assigned NP to coordinate and optimize care. Discuss goals of care and palliative care options; support patients and families in shared decision-making. Promote healthy aging through preventive care and proactive treatment plans. Document accurately and promptly in the EMR; maintain strong clinical documentation habits. Help develop and implement proactive strategies to reduce avoidable hospital transfers and readmissions. Qualifications MD or DO from an accredited medical school; Board Certified/Board Eligible in Internal Medicine, Family Medicine, or Geriatrics. Active, unrestricted state medical license (or eligibility to obtain) and current DEA registration. Experience in post-acute/long-term care, ventilator care, or behavioral health preferred.
