Job Description

Upper Great Lakes Family Health is seeking a Business Transformation Coach to play a critical role in driving organizational effectiveness across a multi-site Federally Qualified Health Center. This position partners with clinical and administrative leaders to project management efforts, facilitate change initiatives, and improve operational workflows - particularly within clinic operations. The Coach will work primarily remotely while traveling periodically to clinic sites for observation, shadowing, stakeholders engagement, and hands-on support to ensure sustainable transformation and adoption of best practices. Responsibilities include but are not limited to: Lead and manage cross-functional projects aimed at improving operational efficiency, patient access, and care delivery. Develop project plans, timelines, milestones, and performance metrics. Track progress, identify risks, and ensure timely completion of initiatives. Coordinate with leadership and clinic teams to align priorities and resources. Serve as a change agent to support adoption of new processes, workflows, and technologies. Facilitate meetings, workshops, and training sessions to drive engagement and buy-in. Assess organizational readiness and provide coaching to leaders and staff. Develop and implement communication and change management plans. Analyze current workflows, identify inefficiencies, and recommend improvements. Apply Lean, Six Sigma, or other process improvement methodologies (as applicable). Standardize best practices across clinic sites while respecting site-specific needs. Monitor key performance indicators (KPIs) and support continuous improvement efforts. Observe and evaluate front- and back-office clinical workflows, including scheduling, patient flow, intake, and care coordination. Partner with clinical teams to improve throughput, reduce wait times, and enhance patient experience. Support optimization of electronic health record (EHR) usage and documentation workflows. Travel to clinic sites to conduct observations, shadow staff, and meet with leadership and teams. Build strong relationships with providers, care teams, and operational staff. Provide hands-on coaching and support during implementation of new initiatives. About Us Upper Great Lakes Family Health is your community-based medical, dental, and behavioral health care center.
