Job Description
Fractional Chief Operating Officer (COO) - Dermatology Practice (Santa Monica, CA) We are a well-established, high-end dermatology and skin surgery practice in Santa Monica seeking a Fractional Chief Operating Officer (COO) to partner closely with the founder and lead the day-to-day operations of the business. This is an in-person, hands-on, strategic + operational role ideal for someone who thrives in a fast-paced, service-driven healthcare environment and enjoys building systems, leading teams, and driving measurable results. About the Practice Established, busy physician-owned dermatology practice with 4 providers Mix of cosmetic, medical, and surgical (including Mohs) services Known for high-touch, hospitality-driven patient experience with highest number of 5-star google reviews in the area Role Overview The Fractional COO will work directly with the owner to optimize operations, improve profitability, reduce inefficiencies, and strengthen team performance while maintaining a high-touch, premium patient experience. This role requires both strategic thinking and execution — you will not only design systems but also ensure they are implemented and followed.
Estimated commitment:
~20-25 hours per week Key Responsibilities 1. Operational Leadership & Oversight Oversee day-to-day operations of front and back office teams Ensure smooth clinic flow, provider support, and patient experience Identify inefficiencies and implement scalable systems and workflows Maintain a high standard of professionalism and accountability across staff 2. Financial & Performance Management Monitor key metrics including: Revenue per provider Schedule utilization Cost of goods (cosmetics, supplies) Payroll as % of revenue Accounts receivable aging Identify revenue leakage (uncollected balances, underutilized schedules, inefficiencies) Partner with billing and accounting teams to improve collections and reduce A/R Provide regular reporting and actionable insights to ownership Use data-driven insights to make informed decisions and drive continuous improvement. 3. Staffing, Culture & Accountability Oversee staffing structure, scheduling, and coverage Address performance issues proactively and professionally Implement clear expectations, KPIs, and accountability systems for team members Support hiring, onboarding, and training processes Foster a positive but high-performance culture 4. Systems & Process Optimization Evaluate and improve existing systems (EMR, scheduling, patient communication tools) Implement standard operating procedures (SOPs) across departments Reduce waste (supplies, time, inefficiencies) Improve coordination between front and back office teams 5. Vendor & Cost Management Review vendor contracts and expenses (software, supplies, services) Identify opportunities to reduce overhead without compromising quality Negotiate and manage vendor relationships 6.Social Media and Marketing:
Oversee and improve the practice's social media presence. Ideal Candidate Profile We are looking for someone who is: Highly operational — you get into the details and make things happen Financially savvy — comfortable creating reports, analyzing numbers and making decisions based on data Great Interpersonal skills — you create a positive work culture and help resolve interpersonnel conflicts Systems-oriented — naturally identifies inefficiencies and builds structure Adaptable and hands-on — willing to step in where needed. No task is too small or too big. Trustworthy and discreet — will have access to sensitive business information Preferred Experience Proven experience as a Chief Operating Officer or similar role in a healthcare setting, preferably within dermatology or a related field.Strong understanding of:
Medical billing and collections Healthcare operations Cosmetic dermatology workflows (preferred but not required) Experience managing teams in a service-based business Familiarity with EMR systems (Modernizing Medicine/EMA a plus) Excellent financial acumen and a strategic thinker with a track record of successful planning and execution. Compensation This is a 1099 fractional role Compensation will be tailored based on experience and include performance-based incentives tied to key operational and financial metrics (e.g., revenue growth, schedule utilization, collections, and expense optimization).
