Job Description

Job Summary The Director of Admissions supports census growth and resident intake for a Skilled Nursing Facility by managing admissions, transfers, and discharges while ensuring compliance with Medicare, Medicaid, and facility policies. This role serves as a key liaison between referral sources, residents, families, nursing, and administrative teams to ensure timely, accurate, and smooth admissions. Key Responsibilities Coordinate SNF admissions, transfers, and discharges in accordance with Medicare, Medicaid, and facility requirements. Receive and process referrals from hospitals, case managers, and other referral sources. Collect, review, and verify admission documentation, authorizations, and resident information. Maintain accurate bed availability, census reports, and resident waiting lists. Communicate admission status and room assignments with nursing, therapy, and interdisciplinary teams. Provide residents and families with admission packets, resident rights, privacy notices, and financial information. Obtain required signatures, consents, and authorizations prior to or upon admission. Ensure admission and medical records are complete and forwarded to appropriate departments. Support admission-related problem resolution in coordination with nursing, social services, and the business office. Maintain confidentiality of protected health information and comply with HIPAA and facility privacy policies. Assist with administrative and clerical tasks related to admissions and census management. Qualifications Prior experience in SNF admissions, healthcare intake, or medical office administration preferred. Working knowledge of Medicare and Medicaid admission requirements strongly preferred. Strong organizational, communication, and follow-up skills. Ability to manage multiple referrals and priorities in a fast-paced SNF environment. Basic computer proficiency, including electronic medical records and data entry systems. Work Environment & Physical Requirements Office-based role within a Skilled Nursing Facility with frequent interaction throughout the building. Regular interaction with residents, families, hospital case managers, and clinical staff.
Pay:
$40.00 - $50.00 per hour
