Participes in root-cause analysis of significant medicion errors. Identifies the extent of the health system's adherence to published best practices and guidelines relevant to medicion-use safety. Legal and Regulory Compliance Functions as a consultant to management personnel regarding pharmacy activities relion Joint Commission, Office of Inspector General, Commission on Accredition of Rehabilition Facilities, etc. Functions as point of contact for medicion safety policies and procedures involving Pharmacy Service. Coordines internal pharmacy quality assurance tracers and educion on Joint Commission and other regulory topics; identifies practice pters for review and targeting. Coordines with pharmacy leadership on service compliance with VA policies and procedures. Participes in policy and procedure updes to ensure compliance of facility. Understands and provides educion on regulory processes, such as controlled substances management. Safety & Storage Assists the Pharmacy Chief in developing medicion safety goals and objectives. Assists in development, approval, and implemention of policies and procedures th support medicion safety. Coordine interdepartmental and intradepartmental medicion safety activities. Ensures parenteral solutions, medicion concentrions, doses, and administrion items are standardized whenever possible. Minimizes the potential for errors associed with medicion products due to labeling, packaging, or medicion names th look or sound alike. Ensures th medicion storage is safe and secure throughout the institution. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

Educion :

(1) Gradue of an Accredition Council for Pharmacy Educion (ACPE) accredited College or School of Pharmacy with a baccalauree degree in pharmacy (BS Pharmacy) and/or Doctor of Pharmacy (Pharm.

D.) degree.

NOTE:

Prior to 2005 ACPE accredited both baccalauree and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy. (2) Gradue of foreign pharmacy degree meet the educional requirements if the gradue is able to provide proof of achieving the Foreign Pharmacy Gradue Examinion Commission (FPGEC) Certificion, which includes passing the Foreign Pharmacy Gradue Equivalency Examinion (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).

Licensure :

Full, current, and unrestricted license to practice pharmacy in a Ste, Territory, Commonwealth of the United Stes (i.e. Puerto Rico), or the District of Columbia. Must maintain current registrion. Pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probionary stus may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.

NOTE:

Experience :

You must have least 1 year of experience equivalent to the GS-12 grade level. Examples of this includes, but is not limited to: reviewing, interpreting, and verifying medicion orders for approprieness; processing and filling medicion orders; interacting with and making recommendions to other clinical staff regarding medicion therapy ordered to ensure safe and effective care; reviewing the pient's medicions, allergies, labs, and other pertinent informion from the medical record to identify and solve medicion-reled problems; contacting providers as approprie; documenting recommendions and interventions.

Preferred Experience:

Prior Medicion Safety and/or Regulory Compliance Program Management experience.
