We are seeking a highly experienced and skilled Medical Director to join our team and provide clinical oversight and support across our facilities. About the
Role:
As a Medical Director , you will be responsible for providing one-time initial patient visits to establish medical oversight and support continuity of care. You will work closely with treating providers to ensure ongoing management and remain available for provider consultation and clinical guidance as needed. This role is ideal for a physician looking for a flexible, travel-based position with meaningful impact and limited ongoing patient panel responsibilities.
Responsibilities:
  • Provide one-time initial patient visits across skilled nursing facilities throughout the state
  • Establish medical oversight and support continuity of care for assigned patients
  • Work closely with treating providers to ensure ongoing management and coordination of care
  • Remain available for provider consultation and clinical guidance as needed
  • Collaborate with the Carease Health clinical team to ensure seamless alignment with facility processes and protocols
