Post-Acute / Long-Term Care About the Role We are seeking a dedicated and experienced Medical Director to lead clinical oversight at our skilled nursing facility in Ardmore, Oklahoma. This is a full-time opportunity for a physician who is passionate about geriatric care and committed to delivering high-quality, compassionate outcomes in a post-acute and long-term care environment. As Medical Director, you will serve as the physician leader of the facility — guiding clinical policy, overseeing care quality, and partnering with nursing, therapy, and administrative leadership to ensure our residents receive exceptional care. Key Responsibilities The Medical Director is expected to be physically present at the facility a minimum of one (1) day per week. Onsite visits should be scheduled to support clinical rounding, care plan meetings, interdisciplinary team collaboration, and direct resident oversight as needed. Provide overall medical leadership and clinical oversight for the facility in accordance with federal and state regulations (CMS, OSDH) Collaborate with the Director of Nursing and interdisciplinary care team on care planning, quality improvement, and clinical protocols Conduct and/or oversee resident assessments, care plan meetings, and complex case reviews Serve as the primary physician of record for residents without an attending physician, as needed Lead or participate in Quality Assurance and Performance Improvement (QAPI) committee activities Ensure compliance with Medicare/Medicaid standards and support survey readiness Mentor and support attending physicians and advanced practice providers (APPs) at the facility
Qualifications Required:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) Active, unrestricted Oklahoma medical license (or ability to obtain prior to start) Board certification in Internal Medicine, Family Medicine, Geriatrics, or a related specialty Minimum 2 years of experience in a skilled nursing, post-acute, or long-term care setting Strong knowledge of CMS regulations, MDS process, and SNF clinical workflows Demonstrated leadership ability and experience working within interdisciplinary care teams
Preferred:
Fellowship or added qualification in Geriatric Medicine (CAQ-G or equivalent) Prior Medical Director experience in a SNF or post-acute setting Familiarity with PointClickCare or similar EHR platforms used in SNF environments CMD (Certified Medical Director) credential through AMDA — The Society for Post-Acute and Long-Term Care Medicine, or willingness to pursue Why Join Us Mission-driven team focused on dignity, quality, and resident-centered care Collaborative environment with experienced nursing and therapy staff Opportunity to shape clinical culture and quality outcomes at a community-based facility Supportive administrative leadership committed to physician success About the Facility The position is at a skilled nursing and rehabilitation facility located in Ardmore, Oklahoma, serving Carter County and the surrounding communities. Residents have access to on-site physical, occupational, and speech therapy delivered by a dedicated interdisciplinary care team.
