The Texas Correctional Office on Offenders with Medical or Mental Impairments (TCOOMMI) Residential Case Manager is responsible for providing outstanding person-centered case management services to adults diagnosed with severe and persistent mental health disorders as well as substance use disorders who are currently housed in the Williamson County Dual Diagnosis Residential Facility (DDRF). The position reports to the TCOOMMI Program Director and requires the development and maintenance of effective, professional working relationships with the DDRF staff, BCBS staff and all outside community partners. This is a Monday through Friday, non-exempt, position with no nightshift or on-call requirements. The TCOOMMI Residential Case Manager (RCM) meets with individuals who are receiving treatment at the DDRF to provide case management, skills training and psychosocial rehabilitative services in collaboration with the Licensed Chemical Dependency Counselors (LCDC) at the facility. Services are intended to stabilize mental health and substance use symptoms, reduce risk of recidivism, increase the individuals awareness of and participation with community and natural supports, develop self-advocacy, and assist with their ability to participate in independent behavioral health services. The person in this position will work in partnership with the Residential LCDCs to develop a treatment plan that will address the identified mental health needs as well as any substance use challenges. This position may also necessitate transportation to the BCBS clinic as well as any other transportation needs identified. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. The person in this position must possess a passion and aptitude for working with individuals who have chronic and severe mental health needs as well as substance used needs and are involved in the criminal justice system. Knowledge of psychotherapeutic medications, case management functions, substance use treatment, crisis services, community resources, and evidence-based practices in the behavioral health field is critical for success in the position. Effective case management employs innovative thinking, patience, resiliency, skills in advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, clinical documentation skills, and dual-diagnosis treatment. The case manager works independently and within a team, prioritizes needs during crisis situations, engages in regular meetings and training opportunities, and performs other duties as assigned with a positive attitude.

Minimum Qualifications:
Bachelors degree in human services.
  • Two years of comprehensive knowledge of mental health and treatment. Ability to work independently and collaboratively within a fast-paced, recovery-focused environment. A demonstrated aptitude to connect with people. Must have a valid Texas Driver's License, meet Bluebonnet Trails Community Services driving requirements, and maintain approved driving status throughout employment.
  • If a degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field are required.
NCIC and TCIC criminal background checks, substance use screening, and fingerprinting are performed prior to employment. An official copy of the college transcript is required.
Preferred Qualifications:
Licensed Masters Social Worker, Licensed Clinical Social Workers, Licensed Marriage and Family Therapist, Licensed Professional Counselor or Licensed Associate. Two years experience in providing wellness and recovery support services in mental health, substance use and/or crisis intervention and screening. Pursuit of clinical licensure in counseling, behavior analysis, family therapy, social work or a related field. Advanced licensure. Bilingual in Spanish. Experience in providing concurrent documentation. Licensure as a Licensed Chemical Dependency Counselor (LCDC).
