Community Resource Specialist Job Responsibilities:
Develop and maintain best practice guidance for Level I assessors and Level II evaluators. Provide technical assistance on individual cases and coordinate referrals to appropriate state and community-based resources. Collaborate with behavioral health and long-term services programs to support community-based care options. Support efforts to reduce unnecessary institutional placement through improved diversion strategies. Create and maintain tracking systems for case support and service documentation. Maintain strict confidentiality and handle sensitive information appropriately.Qualifications:
Community Resource Specialist Job Qualifications:
Bachelor's degree in Behavioral Health or related field required. Previous experience working with individuals with intellectual/developmental disabilities or related populations. Familiarity with nursing facility or behavioral health settings preferred. Strong written and verbal communication skills with ability to synthesize information. Proficient computer skills, including Excel and data tracking systems. Strong organization skills with the ability to manage multiple priorities.
