Job Description

Primary Purpose Collaborate with district staff and outside community partners to provide prevention services under the Prevention and Behavioral Health Promotion Grant Program. Provide direct service to youth, families, and communities using approved evidence-based curriculum education, information dissemination, positive alternative activities, appropriate referrals to community resources, and environmental strategies. Actively engage in actions that contribute to the overall mission and strategic plan of Clear Creek ISD. Qualifications Education/Certification/Licensure High school diploma or GED Bachelor's Degree (preferred) Ability to obtain Associated Prevention Specialist Certification within 20 months of employment Special Knowledge/Skills Strong organizational, communication, public relations, and interpersonal skills Ability to coordinate district functions Ability to organize and maintain data Ability to implement procedures Computer skills consistent with district expectations Must also be able to meet the following training requirements: Cardiopulmonary Resuscitation (CPR) and
First Aid Certifications:
obtained within 60 calendar days of Youth and Adult Mental Health First Aid, obtained within 90 calendar days of Suicide Prevention, obtained within 90 calendar days of Experience Minimum of 3 years successful work experience with school-aged children Experience working with at-risk students and their families (preferred) Major Responsibilities and Duties Complete evidenced-based curriculum training for each YP program funded by HHSC including Curriculum Based Support Group, Positive Action, and Botvin's Life Skills. Document all Reporting Requirements for each small group facilitated by the Alliance ensuring fidelity to curriculum developer requirements. As required by HHSC, implement evidence-based curriculum with fidelity and keep detailed records including pre- and post- Make referrals for students and families as necessary and keep proper documentation of all such referrals. As requested, make Presentations at community sites and on campuses on substance use prevention and behavioral health promotion. As requested, participate in Positive Alternatives at community sites and on campuses. Support CCISD Community Connections Programs at campuses served including, but not limited to, Clear View, Clear Path, and Clear Horizons Early College High School. Stay up to date on current trends regarding substance use and behavioral health promotion in order to ensure presentations have the most current information. Complete INDEPTH Training through the American Lung Association and remain up-to-date as a certified facilitator. Develop and maintain accurate records of services provided. Establish and maintain relationships with community organizations and partners and maintain up-to-date list of community resources. Serve as liaison between students and community services. Participate in community-based processes such as CRCG, SHAC, United Way, and other groups. Maintain all documentation required for HHSC documentation (the PATT and CMBHS) and ensure timely delivery for review. Participate in weekly staff meetings. Demonstrates proficient levels of technology applications. Participates fully in drills and safety exercises to provide for the safety and overall emotional wellbeing of students. Utilize time wisely for effective management of job responsibilities. Maintain punctuality in daily work times, appointments, and meetings. Meet task completion deadlines established by supervisor. Maintain friendly customer-service-driven interactions with all stakeholders, students, teachers, administrators, and co-workers. Work cooperatively with co-workers and supervisors to ensure that the goals of the school/department are met. Maintain a positive and professional tone in all communication (i.e. email, written, and verbal). Perform other duties and accept other responsibilities as assigned.
