Employment is contingent upon external funding and the continued availability of these funds. MISSION The mission of Ravenswood Family Health Network (RFHN) is to improve the health of the community by providing culturally sensitive, integrated primary and preventative health care to all, regardless of ability to pay or immigration status, and collaborating with community partners to address the social determinants of health.
POSITION
Summary The School Integration Team Lead, as a member of the pediatric care and operations team, provides leadership for RFHN's School Integration Team (SIT) to expand a proven clinic-school partnership model across early childhood centers and elementary schools in East Palo Alto and Belle Haven. Anchored at RFHN's East Palo Alto location, the SIT extends medical home services into school settings in a primarily virtual manner while maintaining a consistent on-site presence to reduce barriers to care and address health barriers to learning. Reporting to the Associate Medical Director for Pediatrics and the Pediatrics Operations Manager, the Team Lead is responsible for planning, launching, and scaling pilot school partnerships over a four-year grant period. This includes overseeing development of standardized, bidirectional release-of-information processes between Ravenswood City School District and medical homes (in alignment with HIPAA, FERPA, and other applicable statutes), coordinating preventive health screenings and integrated services, strengthening care coordination and family engagement, and using data to demonstrate improvements in health outcomes and chronic absenteeism. The Team Lead supervises and supports Community Health Fellows embedded at partner schools and coordinates closely with internal clinical staff (e.g., Pediatric Director, Nurses, Medical Assistants, and Virtual Dental Home team) and external school/community partners to ensure high-quality implementation, continuous improvement, and progress toward long-term sustainability aligned with Medi-Cal/CalAIM reimbursement pathways. Duties and Responsibilities To be performed in accordance with RFHN Policies and Procedures Management (40%) Recruit and hire a total of 3 school integration fellows over the grant period. Provide direct supervision to fellows including training, oversight, scheduling, and all personnel management needs (performance review, time card management, etc.) in coordination with our HR, Payroll, Compliance, IT, and other departments. Assist in the professional development of fellows as it relates to their knowledge of pediatric preventive and general medical services, primary education environments, and use of EHR and related systems to further health promotion within school environments. Operations (30%): Define the structure and approach of the school integration team to meet the yearly goals of our grant. Develop any needed policies or procedures for the SIT. Attend regular meetings with and provide regular updates to operational and clinical management for the Pediatric and Family Practice departments along with other RHFN leadership. Monitor grant deliverables, timelines, and ensure these are met. Work regularly with our Development team on reporting and budgetary concerns related to grant implementation. Identify, collect, and analyze data related to SIT performance, outcomes, and impact on health care expenditures (in conjunction with other partners such as Medi-Cal). Advocacy and Community Partner development (30%): Establish and maintain productive community partner relationships with stakeholders among our educational partners (RCSD, Kidango, early learning), community-based services partners (CHC), payor and governmental groups (HPSM, GGRC, SARC), and others. Directly liaise with legal and advocacy partners on development of release of information exchange procedures. Oversee the construction of bidirectional release of information processes that integrate multiple stakeholders and incorporate any need information technology (IT) components including security assessments, vendor assessment coordination. Perform other duties as assigned to support the mission and operations of
RFHN. QUALIFICATIONS
Bachelor's or equivalent degree required, graduate degree a plus (ie Master's in Public Health, Master's in Education). Experience in clinical environments or prior clinical training a plus but not required. Familiarity with primary educational environments and systems preferred (i.e. preschool, elementary, middle school). Computer skills, including Microsoft Excel, Word, and PowerPoint. Proficiency with electronic health records, Epic a plus. Bilingual Spanish/English proficiency required. Must be able to travel between clinical locations and school sites as required. Up to date with
COVID-19
vaccines per current CDC guidelines strongly recommended. The pay range for this position is $120,000.00.00 to $140,000.00 per hour. However, the final base pay will be determined upon a number of individualized factors such as (but not limited to) the scope and responsibilities of the position, job-related knowledge, skills, experience, education and certification levels, and departmental budget.
