Job Description

Case Manager Full-Time Fall River, MA and New Bedford, MA The Emergency Solutions Grant (ESG) program of Catholic Charities of the Diocese of Fall River, provides funding to homeless individuals, helps improve the number and quality of emergency shelters, helps operate shelters, provides essential services, rapidly re-houses homeless individual and families and prevents individuals and families from becoming homeless. The main focus of the ESG program is to provide assistance to individuals and families to quickly regain stability in permanent housing after experiencing a housing crisis and/or homelessness. Additionally, Catholic Charities connects families to resources that are available within their respective communities to sustain long-term stable housing. The ESG Case Manager will focus on the coordination and delivery of services, including direct financial assistance, case management, reporting, and community outreach as well as work with low-income families who are facing evictions to prevent homelessness and assisting clients to move from homelessness (including shelters) into stable housing. The Case Manager is also responsible providing services that are safe, timely, effective, efficient, equitable and client-centered.
Qualifications include:
BA/BS in Human Services preferred, experience in human services, housing and homelessness services Must be comfortable working in community settings High level of personal accountability for the quality and impact of work Reliable transportation required Bi-lingual helpful Why join our team??
