As a Case Manager, you play a critical role in supporting veterans and their families by providing comprehensive case management and suicide prevention resource navigation. This position manages a caseload of approximately 25-30 participants and serves designated counties across Oklahoma. Guided by the U.S. Department of Veterans Affairs National Strategy for Preventing Veteran Suicide (2018) , the Case Manager provides assessment, planning, coordination, advocacy, and follow‑up services to ensure veterans and their families are connected to the care and resources they need. This is a mission‑driven role ideal for professionals passionate about veteran services, behavioral health support, and community‑based care coordination. What You'll Do Serve as first or second point of contact for eligible veterans and families Complete eligibility, assessments, and individualized service plans, and maintain documentation Provide bi‑weekly case management meetings (in‑person, phone, or virtual) Connect veterans to VA mental health care and community resources Support employment, housing, benefits, behavioral health, and basic needs Provide weekly check‑ins and 3-6 months of follow‑up/aftercare Collaborate with community and veteran service organizations Attend required meetings, trainings, and outreach events What We're Looking For High School Diploma or equivalent. 2+ years related experience (case management preferred) Knowledge of veteran services, behavioral health, or social services Strong communication, organization, and time‑management skills Comfortable using Microsoft 365 and online case management systems Additional Requirements Ability to travel within assigned counties Valid driver's license Occasional statewide or conference travel Why You'll Love This Role Meaningful, life‑saving work Strong team support and collaboration Ongoing training and professional development Mission‑focused nonprofit environment
