A beautiful hospital near Breckenridge, CO is seeking a full-time, permanent Transitional Care Nurse Coordinator to join their growing team. More details below!
Pay:
$90,000.00-$130,000.00/year + Performance Bonuses Relocation Assistance is
Provided Transitional Care Nurse Coordinator Opportunity:
Full-time, direct hire position
Schedule:
Monday-Friday, 9a-5p Transitional Care Nurse Coordinator will lead and implement the swing bed program, acquire hospital referrals and assess them for their quality and cost efficiency, and oversee the admission and discharge of referred patients Given an expense account and travel budget to travel to nearby areas in order to expand relationships and referral opportunities Generous PTO, retirement plan, relocation assistance, and more!
Transitional Care Nurse Coordinator Requirements:
Must hold a Bachelor's degree in Nursing Active, unencumbered RN License Reliable transportation, a valid driver's license, and vehicle insurance are required 2+ years of experience as a Transitional Care Nurse Coordinator, case manager, or patient navigator Experience in rural communities and healthcare sales/marketing is preferred
