JOB SUMMARY
As the Activities Assistant, you'll assist and support the Director of Activities in planning, organizing, developing, and directing quality activities for patients, ensuring that the recreational, physical, intellectual, spiritual, and social needs of each patient is met in accordance with all applicable laws, and regulations. In the Resident Transportation Specialist role, you'll play a vital part in enhancing the daily lives of our residents. In this position, you'll safely and respectfully transport individuals to and from medical appointments, therapy sessions, and community outings, ensuring safety, comfort and dignity every step of the way.
JOB DUTIES
Inspects the facility vehicle each day to ensure it is safe for resident transportation Assists in ensuring the facility vehicle's maintenance is completed in accordance with manufacturer guidelines and as needed on a routine basis Ensures residents are transported in a safe manner, following all vehicle code requirements and posted guidelines Ensures residents are properly secured prior to and during transportation May assist resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. May assist residents with or performs activities of daily living for resident in accordance with care plans and established policies and procedures Observe resident mood and behavior and report any changes or concerns to the Activities Director Coordinate transportation needs and requests with other departments in a courteous and timely manner Reports changes in a resident's condition to the resident's nurse and supervisor Treats all residents with dignity and respect Promotes and protects all residents' rights Establishes a culture of compliance by adhering to all facility policies and procedures Complies with standards of business conduct, and state/federal regulations and guidelines Follows appropriate safety and hygiene measures at all times to protect residents and themselves Maintains confidentiality of protected health information, including verbal, written, and electronic communications Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel Reports any retaliation or discrimination to the Administrator Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to the Administrator. Protects residents from abuse and cooperates with all investigations Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facility's emergency plan Reports work-related injuries and illnesses immediately to supervisor Follows established infection control policies and procedures As a condition of employment, completes all assigned training and skills competency As a condition of employment, maintains a clean driving record, without substantial violation(s), throughout term of employment
REQUIREMENTS / EDUCATION / EXPERIENCE
A current and unencumbered driver's licenses in the state Class B with passenger endorsement
  • preferred Certified Nursing Assistant in good standing with the state
  • preferred Must be a high school graduate; Associate degree
  • preferred Crisis Prevention Intervention (CPI)
  • preferred Motivational interviewing
  • preferred Behavioral Therapy
  • preferred Able to work swing shifts and weekends Current CPR/BLS certification
  • preferred Trained in First Aide care and response Must have no disciplinary action in effect against certification Must be able to fluently read, write, speak and understand English language Must be a supportive team member, contribute to and be an example of teamwork Ability to make independent decisions when circumstances warrant such action Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public Must have patience, tact, and willingness to deal with difficult residents, family and staff.
Must not pose a threat to the health and safety of other individuals in the workplace Must be able to move throughout the workday Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases Ability to assist in evacuation of residents during emergency situations Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position May be subject to falls, burns from equipment, and/or odors throughout the day; encounter reactions from dust, tobacco smoke, disinfectants, automotive fuels and chemicals and other air contaminants Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis May be subject to hostile or emotional residents, family members, visitors or personnel Effective verbal and written communication skills and ability to exercise judgement Ability to maintain composure in stressful situations Resourcefulness Strong leadership skills Maintain a positive attitude and possess the ability to work in stressful situations Ability to be organized and efficient Must possess the ability to make independent decisions when circumstances warrant such action Must have the ability to implement and interpret the programs, goals, objectives, policies, and procedures of the department Must perform proficiently in all competency areas including but not limited to: patient rights, and safety and sanitation Maintains confidentiality of all proprietary and/or confidential information Must understand and follow company policies including harassment and compliance procedures Displays integrity and professionalism by adhering to Storybrook Care and Rehabilitation Center Code of Conduct and completes mandatory Code of Conduct and other appropriate compliance training Complete any other tasks assigned by Activities Director and/or Management Affirmative Action Statement Storybrook Care and Rehabilitation is an Equal Opportunity Employer committed to fostering a diverse and inclusive workplace.
