Part-time:
20-25 hours per week
Location:
Berlin, CT Why This Role Feels Different This role bridges school and real life. You'll work directly with students, families, and community partners to build individualized transition plans that lead to meaningful post-secondary outcomes. What You'll Do Develop and implement transition plans aligned with IEP goals Conduct transition assessments to identify strengths and interests Support students in career readiness and independent living skills Build relationships with community agencies and local employers Educate families and staff on transition resources and pathways What You Bring Bachelor's degree in Special Education, Counseling, or Human Services CT Certification (165, 068, or 071) Experience supporting students with disabilities Knowledge of community resources and vocational services Why Prism Flexible, part-time schedule Sick time and referral program Highly collaborative, mission-driven environment Let's Build Something Meaningful If you're passionate about helping students take their next step with purpose, we'd love to connect.
