Job Description

Position Summary:
Continuum Care Services, Inc. is seeking a reliable, safety-conscious Transportation Specialist to provide non-medical, non-emergency transportation to patients/consumers for scheduled appointments and activities. This role is essential in supporting the well-being and independence of the individuals we serve.

Will be determined based on experience
Key Responsibilities:
Safely transport patients/consumers to and from appointments and activities Monitor client safety and behavior during transport Communicate clearly with scheduling staff and care teams regarding transport times and outcomes Ensure vehicle cleanliness and report maintenance issues promptly Adjust routes/schedules as needed based on client needs and team direction Support and promote client rights, dignity, and choice Follow all safety, infection control, and HIPAA/confidentiality guidelines Maintain accurate transport logs and documentation
Required Education:
High School Diploma or Equivalent Occupational Certification/Licenses:
Valid and unrestricted NC driver's license and personal vehicle insurance
Qualifications:
Must be at least 21 years of age Must pass background and driving record checks Must be drug-free and able to pass applicable screenings Strong communication, reasoning, and interpersonal skills Physically able to sit, stand, walk, and lift medium to heavy objects as needed
Preferred Skills Experience:
Experience coordinating transport schedules Familiarity with working in health or human services settings Previous experience working with individuals with disabilities or mental health needs
Work Environment:
Field-based role with transport in community and home environments Exposure to varying home conditions, weather, and occasional health-related risks Travel between sites and training locations required Why Continuum Care Services Our company is based on the belief that our customers' needs are of the utmost importance.
