This position is responsible for developing referral sources, facilitating patient admissions, coordinating transitions of care, and promoting the facility's services within the healthcare community. The Hospital Liaison ensures timely communication and exceptional customer service while supporting census growth and quality patient outcomes. Essential Duties and Responsibilities Develop and maintain strong relationships with hospital discharge planners, case managers, physicians, social workers, and other referral sources. Conduct routine hospital visits to identify potential admissions and educate healthcare professionals about the facility's services and capabilities. Coordinate patient referrals and admissions, ensuring all required clinical and financial information is obtained and reviewed. Collaborate with the Admissions, Nursing, Therapy, and Clinical and other interdisciplinary teams to facilitate smooth transitions of care. Communicate regularly with patients, families, and referral sources regarding admission status and care options. Represent the facility at healthcare networking events, community outreach activities, and professional meetings. Monitor referral trends and market opportunities to support census development goals. Maintain accurate records of referral activities, admissions, and marketing efforts within designated systems. Ensure compliance with all federal, state, and facility regulations related to admissions and marketing practices. Provide exceptional customer service to referral partners, patients, families, and internal stakeholders. Assist with facility tours and presentations for prospective residents and families. Participate in strategic planning initiatives to enhance facility visibility and referral growth. Qualifications Education Bachelor's degree in Healthcare Administration, Marketing, Business, Social Work, Nursing, or related field preferred. Equivalent combination of education and experience may be considered. Experience Minimum of 2 years of experience in healthcare marketing, admissions, business development, case management, discharge planning, or related healthcare setting preferred. Experience in Skilled Nursing Facilities, Rehabilitation Centers, Home Health, Hospice, or Hospital settings strongly preferred. Knowledge of post-acute care services and healthcare reimbursement systems preferred. Skills and Competencies Strong relationship-building and networking abilities. Excellent verbal and written communication skills. Ability to work independently and manage multiple priorities. Strong organizational and time management skills. Knowledge of Medicare, Medicaid, managed care, and insurance processes. Proficiency in Microsoft Office and electronic healthcare systems. Ability to analyze referral data and identify growth opportunities. Professional presentation and customer service skills. Physical Requirements Frequent travel to hospitals, physician offices, and healthcare facilities. Ability to sit, stand, walk, and drive for extended periods. Ability to occasionally lift up to 25 pounds. Schedule Full-Time Monday-Friday, with occasional evenings or weekends as needed for events, meetings, or referral activities.
