Responsibilities IOP Coordinator Horizon Health is seeking an IOP coordinator for our IOP program at Johnson Regionalin Clarksville, AR. The IOP coordinator provided direct high quality psychotherapy services to high functioning patients in assessments, diagnoses, treatment planning, group treatment, individual treatment, family treatment, and discharge planning for the Intensive Outpatient Program, and also plans and implements programs to meet the social and emotional needs of the patients. This position may fulfill the role of supervisor for the program in the absence of the Program Director and/or during weekend shifts This position will work Monday-Friday 8a-4:30p. There are no weekend or major holiday requirements.
Responsibilities:
Complete psychosocial assessment, Plan of Action, and Plan of Action updates within established timeframe
Develops and implements program plans for patient care during treatment and release from the program
Conduct daily group therapy sessions. Conduct individual and/or family therapy session.
Obtain insurance benefit information, authorization for treatment and perform ongoing utilization reviews.
Provides comprehensive education, instructions, and/or advocacy information designed to assist the patient and significant others to identify secure needed health care follow-up
Plans and evaluates department social service activities; communicating the plans to program stakeholders
Works in partnership with the Program Director, community-based providers, Horizon leadership, client hospital administrative staff and clinical staff to develop and implement programs that promote Unit behavioral health programs within the area/community
Maintains compliance with state and federal confidentiality laws as well as upholding Horizon's confidentiality policies and procedures
Assesses social services program and patient progress; communicating results and trends to the Program Director and treatment team
Conducts individual, group, and family psychotherapy sessions in accordance with state/regulatory standards, displays sufficient theoretical knowledge of psychopathology and effective treatment techniques
Develops, implements and participates in a 24-hour call process to handle emergency situations
Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources, and makes effective contact with disposition sources
Submits timely and accurate CQI+ outcome data on a weekly basis
Maintains all required state/regulatory, hospital and company competencies and training expectations
Participates as an active member of the Referral Development Team
