General Summary:
The Foster Care Program Coordinator I supports community foster home providers in implementing basic behavior interventions and supervises foster parents ensuring safety and compliance of foster care environment. This role is critical in supporting the well-being of children in foster care and ensuring that foster families are equipped to provide a nurturing and safe home.Job Responsibilities:
Home Visits In coordination with DCS and foster family, monitor and record behavior problems identified in the home, school and/or community. Provide guidance and support to foster parents on effective strategies to address behavior problems identified. In coordination with DCS refer foster child and/or family to behavioral health outpatient services. Reports critical incidents within contractual time frames to Supervisor. Document all required documentation (service planning, interventions and incidents) in the EHR. Supervision of Foster Parents During home visit assess the living environment, safety issues, and ensure adherence to foster care regulations and organization goals. Document all home visits and contacts in theEHR/OLR/DCS.
Train and assist foster parents on using provider portal to upload required documents. Offer training and resources to foster parents, enhancing their skills in providing care and support to foster children. Evaluate foster parent performance and provide constructive feedback. Safety and Compliance Ensure that all foster homes comply with state and federal regulations regarding safety, health, and welfare standards. Assist foster parents in reporting required progress notes and other documents required. Assist in re-licensure process of foster parents collecting and/or guiding foster parents through the process. Respond promptly to any safety concerns or incidents, taking necessary actions to protect children in care. Address immediate licensing violations to foster parent. Collaboration and Communication Work closely with DCS, therapists, and other stakeholders to ensure a holistic approach to care. Works with Supervisor/Director and Foster Care Compliance Manager assisting in relicensure and maintenance of training records. Participates in supervision with Supervisor and other team members providing updates on foster home. Participate in team meetings and maintain professional relationships with internal and external stakeholders. Other Promote awareness of foster care needs and assist in the recruit of potential foster families as needed. Performs other duties as assigned and in relation to foster care program.Qualifications and Experience Minimum Education Required :
High School Diploma or Equivalent.Minimum Experience Required:
Minimum of 2-years related experience in childcare, foster care or care coordination.License Requirement:
N/A Company / Regulatory Requirements Minimum 21years of age. Able to pass a minimum 5 panel drug test & submit to regular screening. DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment). Maintain a clear Central Registry Background Check. CPR, First Aid, (must possess upon hire and maintain throughout employment). Current, valid Arizona Driver's License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements. Questions about this position?
