Costa Mesa; Midway City (temporary) SUMMARY The Program Manager position is responsible for providing strong leadership to ensure the provision of high quality among staff and across departments and programs. As part of a multi-disciplinary team, the Program Manager will travel to multiple locations to meet and work with clients to attain wellness and recovery and sustain permanent housing. The Program Manager will provide supervision and case management services to ensure housing stability, improved health outcomes, increased wellness and recovery, and community integration. This position is responsible for maintaining the highest quality provision of direct services while fostering internal teamwork and continuity of care with other service provider partners.
ESSENTIAL DUTIES & RESPONSIBILITIES 1.
Work directly with the Director of Housing Services to implement program goals. 2. Ensure that services to clients are reflective of AFH's model, which emphasizes customer choice, harm reduction, and psycho-social rehabilitation. 3. Provide support to the staff for emergencies and crisis situations. 4. Be available to respond to emergency calls after work hours and weekends. 5. Assist in the screening, assessment, and enrollment of clients, and determining eligibility. 6. Ensure Case Managers enter weekly case notes on HMIS, Exym and complete CES intakes for new clients. 7. Ensure all clients' paperwork and data collection is complete, timely, accurate, and current in agency records and electronic databases through on-going audits of electronic databases and chart reviews. 8. Ensure housing performance standards of partners and funders are achieved. 9. Provide weekly individual and team supervision, support, and performance evaluations for assigned staff. 10. Provide input, evaluation, and updates regarding programs as requested. 11. As needed, assist in involving clients in daily activities, budgeting, facilitating socialization, health maintenance, sense of community, and life skill workshops and progress in recovery. 12. Maintain confidentiality of clients' files per agency policies and HIPAA and follow all Policies and Procedures as outlined by
HUD, VA, HCA, OCHMIS
contracts or any other contracts. 13. Coordinate and facilitate regular meetings with partner agencies that provide services to members at the site(s). 14. This position will liaise with other community providers to secure additional services and community support for the clients. 15. Participate in funder/program meetings to provide updates and program progress. 16. Build a strong team through open communication and by collaborating on decision-making responsibilities.
QUALIFICATIONS 1.
Bachelor's Degree in related field (Social Work, Psychology, or other Human Services) OR four years supervisory experience in a related field. 2. Minimum of four years' full-time experience working with the homeless, preferably those living with mental illness and/or substance addictions. 3. Current, valid California Driver's License with an acceptable driving record and reliable vehicle. 4. Able to transport clients in your vehicle. 5. Able to provide direct supervision and management of assigned staff. 6. Detail oriented with excellent time management, organizational, communication, interpersonal, and computer skills. 7. Experience with compiling and following strict budgets. 8. Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers. 9. Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision. 10. Strong interpersonal and team building skills. 11. Ability to relate effectively to diverse groups of people from all social and economic segments of the community. 12. Self-starter with the ability to stay ahead of the curve and thrive in fast-paced environment. 13. Able to obtain and maintain CPR/1st Aid Certification. 14. Able to work late hours and weekends. 15. Able to be on-call 24/7. Preferred Qualifications 1. Masters in Social Work, Psychology, or other Human Services field. 2. Two years' supervisory experience. 3. Knowledge and experience in chemical dependency and substance abuse treatment. 4. Minimum 30 WPM typing speed.

WORK ENVIRONMENT 1.
Field and indoor office environment. 2. Will necessitate working in busy and loud environments. 3. Will be exposed to elements like cold, heat, dust, noise, and odor. 4. May need to bend, stoop, twist, and sit throughout the day. 5. Regularly required to walk or drive to different local sites throughout the day. 6. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds.
Pay:
$75,000.00 - $82,500.00 per year
